    Abstract [en]

    Aim of the study. To explore the content and structure of communication between patient and nurse at follow-up appointments concerning hypertension. Background. Hypertension is a chronic condition and calls for co-operation between health care providers and patients over a long period of time. One important purpose of the follow-up consultations is to transfer knowledge between patients and health care providers in order to empower patients. This is an important determinant of the quality of care. Design/methods. The study was based on 20 audio-recordings of actual follow-up appointments and was approved by ethics committees. The consultations took place at four different health care units for hypertensive patients. Findings. The average length of consultations was 18 minutes. In the consultations, patients initiated an average of eight new topics and nurses an average of 20. All nurses talked with patients about life style. Compared with previous studies of follow-ups with physicians, consultations with nurses addressed lifestyle factors and adherence to treatment to a higher degree. It was also observed that patients were more actively involved in interaction with nurses compared with the follow-ups with physicians. Conclusions. Active patient participation in care is a critical factor in improving adherence to treatment. It would be of value to develop and assess a more patient-centred. organization of hypertension care and thereby more individualized hypertension treatment. Nurses may have a pivot role in such care.

    Abstract [en]

    Aim To describe experience from a behavioural medicine intervention among poorly controlled adult type 1 diabetes patients, in terms of feasibility, predictors and associations of improved glycaemic control. Methods Data were collected on 94 poorly controlled adult type 1 diabetes patients who were randomised to a study evaluating the effects of a behavioural medicine intervention. Statistics covered descriptive and comparison analysis. Backward stepwise regression models were used for predictive and agreement analyses involving socio-demographic and medical factors, as well as measures of diabetes self-efficacy (DES), diabetes locus of control (DLOC), self-care activities (SDSCA), diabetes-related distress (Swe-PAID-20), fear of hypoglycaemia (HFS), well-being (WBQ), depression (HAD) and perceived stress (PSS). Results The participation rate in the study was 41% and attrition was 24%. Of those patients actually participating in the behavioural medicine intervention, 13% withdrew. From the regression models no predictors or associations of improvement in HbA1c were found. Conclusions The programme proved to be feasible in terms of design and methods. However, no clear pattern was found regarding predictors or associations of improved metabolic control as the response to the intervention. Further research in this area is called for.

    Kritiskt sjuka barn som vårdades på intensivvårdsavdelning väckte svåra känslosamma reaktioner hos föräldrarna. Syftet med denna studie var att belysa föräldrars upplevelse av omvårdnaden av sitt barn på intensivvårdsavdelning. Metoden som användes var litteraturstudie och omfattade 16 vetenskapliga artiklar. Litteratursökningen gjordes med hjälp av MESH-termerna: intensive care, critical care, parent, experience och need via databaserna PUBMED och CINAHL. Resultatet visade att ovissheten, barnets förändrade utseende och smärta hade betydelse för hur föräldrarna upplevde barnets sjukdomstillstånd. Procedurer, miljön och medicinteknisk apparatur var viktiga faktorer för hur föräldrarna upplevde vården på barnintensivvårdsavdelning (BIVA). Föräldrars behov under BIVA-vistelsen var information, närhet och stöd. Föräldrars upplevelser på BIVA, på barnavdelningen och i hemmet fanns beskrivna. I diskussionen togs det upp att föräldrar som såg sitt barn ha ont, tappade sin föräldraroll eller var oroliga för att barnet inte skulle överleva löpte större risk för ohälsa. Med enkel, tydlig information och möjlighet att vara delaktig och nära sitt barn kunde den kritiska situationen upplevas som mindre stressande för föräldrarna.

    Abstract [en]

    The most common reason for children to be seek in-hospital care in Sweden is trauma and every year more than 200000 children is traumatized. Nurses meet these children in their work but the procedures used to care for traumatized adults are not transferable to children. The aim of this study was to illuminate how special educated nurses experience the caring for traumatized children. A qualitative design with one-to-one interviews (n=9) was used to collect data. Content analysis was used to analyze the data. The analysis ended up in four categories;

    the experience of lacking knowledge, the experience of difficulties in the care, the experience of the need of good nursing skills and the experience of differences in the routines in use. The findings show that all the interviewed nurses experience a need to increase their knowledge concerning the care for traumatized children. Increased skills and knowledge in this matter would raise their feelings of being secure in their professional role.

    Abstract [en]

    Triage, as a concept, is relatively new in Sweden and means 'sorting'. The triage process was developed to grade patients who needed immediate care. Triage is currently important for the emergency treatment system, and nurses are expected to work with it professionally. The aim of this study is to describe how nurses implement triage when patients arrive at the emergency department of a county hospital, situated in a rural area of Sweden, as well as to highlight the factors considered when prioritizing, in connection with nurses' decision-making. The method used was observations of 19 nurses, with minimal disturbance in their triage work, followed by a short tape-recorded interview, during which the nurses were asked to reflect upon their decision of priorities. Qualitative content analysis of data has been used. The results were divided into two areas, internal factors and external factors. The internal factors reflect the nurse skills and personal capacity. The external factors reflect work environment, including high workload and practical arrangements, and should always be perceived and taken into consideration. Using these factors as a basis, the patients' clinical condition, clinical history, various examinations and tests form an assessment, which subsequently results in a prioritization.

    Introduktion: Barn i ung ålder är beroende av att föräldrarna tar hand om barnets astma. Föräldrar ska skapa en omsorgsfull och trygg omgivning för deras barn. Ett barn med astma involverar oftast hela familjen vilket innebär en förändring. Syfte: Syftet med denna litteraturstudie var att beskriva föräldrars erfarenheter att leva med ett barn med astma. Metod: I denna litteraturstudie användes 25 artiklar som analyserades och samanställdes av båda författarna. Resultat: Resultatet visade att barnets astma skapade många olika känslor hos föräldrarna, framförallt oro. Föräldrarna hade brist på kunskap och samlade därför information från olika källor. De skapade olika strategier för att lära sig hantera barnets astma. Föräldrarna hade svårt att lämna över ansvaret av sitt barns astma till andra. Föräldrarna kände sig utelämnade av vårdpersonalen i behandlingen av barnet och kände besvikelse över vårdpersonalens bristande stöd till familjen. Diskussion: Sjuksköterskan bör i mötet med föräldrar uppmuntra och stödja föräldrarna så deras självförtroende i vården av deras barn stärks och påminna dem om de resurser de har. Det är viktigt att sjuksköterskan bjuder in föräldrarna i vården, då de har en viktig roll i behandlingen av barnet. Slutsats: Vi skulle vilja se större utbud och utveckling av bra utbildning till föräldrar och barn med astma, samt vilja se fler studier gjorda i Sverige.

    Introduktion:

    Att få ett besked om att sitt barn har drabbats av cancer är ett förödande besked för hela familjen. Därför behöver sjukvården se till hela familjens behov och ge dem stöd och hjälp för att kunna hantera situationen. Syfte: Syftet med litteraturstudien var att beskriva föräldrarnas upplevelse och reaktion när deras barn får en cancerdiagnos samt vilka strategier de använder sig av för att hantera det. Metod: I denna litteraturstudie ingår 14 stycken artiklar varav 12 stycken är kvalitativa och 2 stycken är kvantitativa som granskades och sammanställdes. Resultat: Föräldrarna reagerade på diagnosen med stor sorg, rädsla och ångest. De beskrev en stor känslomässig stress och förnekelse. Stöd var viktigt för föräldrarna och det kunde vara från familj, vänner, sjukvårdspersonal och föräldrar som är i samma situation. Föräldrarna beskrev också olika strategier som de använde sig av för att hantera situationen, det kunde vara allt från att hålla en positiv attityd till att ta en mycket aktiv roll i barnets vård och tillstånd. Diskussion: Den rädsla, oro och sorg som uppstår hos föräldrarna ger upphov till lidande för dem. Detta lidande är ingenting som kan elimineras utan är något som sjukvården bör försöka lindra och hjälpa föräldrarna igenom. Genom att finnas till hands och tillgodose deras behov på bästa sätt så kan sjukvården underlätta för föräldrarna i en svår situation. Slutsats: Med denna litteraturöversikt kan sjukvården lättare sätta sig in i föräldrarnas tankar och känslor utifrån deras perspektiv när deras barn drabbas utav cancer. Därför kan sjukvården möta föräldrarnas behov och därmed lättare hjälpa dem hantera situationen.

    Bakgrund: Trycksår är ett vanligt problem inom sjukvården som skapar stort lidande för dedrabbade. Trycksår är skador i huden och/eller underliggande vävnad, vanligtvis där benligger nära huden. Detta uppstår främst som resultat av tryck eller tryck i kombination medskjuvning. Syfte: Syftet var att belysa sjuksköterskans preventiva arbete för att förebyggatrycksår samt sjuksköterskans behov för att kunna förebygga trycksår. Metod: Totaltinkluderades 15 artiklar i litteraturöversiktens resultat. Artiklarna valdes ut systematiskt föratt finna studier som svarade till litteraturöversiktens syfte. Artiklarna som valdes ut varkvalitativa, kvantitativa och av mixad design. Resultat: Sjuksköterskans arbetsuppgifter idet preventiva arbetet mot trycksår inkluderade samarbete med andra professioner,dokumentation, utförande av riskbedömningar samt att tillgodose patienternas behov avnäring och utförande av hudvård. Sjuksköterskan försökte även få patienterna delaktiga i sinvård genom att ge rikligt med information. För att göra detta möjligt hade sjuksköterskan ettbehov av adekvat utrustning som exempelvis tryckreducerande madrasser, glidlakan ochkilkuddar samt utbildning för att bibehålla kunskap inom området.Diskussion: Sjuksköterskor behöver adekvat utbildning och utrustning för att bedriva engod omvårdnad och ett trycksårsförebyggande arbete som främjar patientens förutsättningarför att bli oberoende. Slutsats: Fortbildning samt kvalitetssäkrad utrustning kan ses som enförutsättning för sjuksköterskans preventiva arbete mot trycksår. Framtida forskning börfokusera på mer delaktiga patienter i form av egenvård för ett trycksårsförebyggande arbete.

    Bakgrund:

    Våld mot kvinnor är ett universellt och allvarligt folkhälsoproblem som påverkar kvinnors fysiska och psykiska hälsa samt kränker hennes mänskliga rättigheter. Vanligast är att bli utsatt för våld av sin partner och globalt har 38 % av morden på kvinnor begåtts av en partner.

    Syfte:

    Att belysa sjuksköterskans perspektiv på bemötandet av den våldsutsatta kvinnan.

    Metod:

    En allmän litteraturöversikt där elva vetenskapliga artiklar har granskats och sammanställts.

    Resultat:

    Sjuksköterskan upplevde rädsla och osäkerhet i mötet vilket bidrog till att det var svårt att fråga kvinnan om våld. När kvinnovåldet bekräftades framkallades olika känslor hos sjuksköterskan och hon blev känslomässigt involverad. Sjuksköterskan agerade i situationen genom att vidta olika åtgärder.

    Diskussion:

    Ett bra bemötande bygger på att våldsutsatta kvinnor behöver bli behandlade med respekt och bli tagna på allvar. Sjuksköterskans bemötande betraktas som betydelsefullt eftersom ett dåligt bemötande kan leda till att patienter undviker att söka vård och lidandet kan därmed kvarstå.

    Slutsats:

    Ett sätt att minska sjuksköterskornas osäkerhet kan vara att införa simuleringar, handlingsplaner, screening, utbildningsdagar eller införa ämnet i sjuksköterskeutbildningen. Detta kan stärka självsäkerheten hos sjuksköterskor och övrig vårdpersonal att våga bemöta problemet.

    Abstract [en]

    Objective The aim of this study was to compare women's satisfaction with group based antenatal care and standard care.

    Design A randomised control trial where midwives were randomized to perform either GBAC or standard care. Women were invited to evaluate the two models of care. Data was collected by two questionnaires, in early pregnancy and six months after birth. Crude and adjusted odds ratios with a 95% confidence interval were calculated by model of care.

    Settings Twelve antenatal clinics in Sweden between September 2008 and December 2010.

    Participants Women in various part of Sweden (n=700).

    Findings In total, 8:16 variables in GBAC versus 9:16 in standard care were reported as deficient. Women in GBAC reported significantly less deficiencies with information about labour/birth OR 0.16 (0.10–0.27), breastfeeding OR 0.58 (0.37–0.90) and time following birth OR 0.61 (0.40–0.94). Engagement from the midwives OR 0.44 (0.25–0.78) and being taken seriously OR 0.55 (0.31–0.98) were also found to be less deficient. Women in GBAC reported the highest level of deficiency with information about pregnancy OR 3.45 (2.03–5.85) but reported less deficiency with time to plan the birth OR 0.61 (0.39–0.96). In addition, women in GBAC more satisfied with care in supporting contact with other parents OR 3.86 (2.30–6.46) and felt more support to initiate breastfeeding OR 1.75 (1.02–2.88).

    Conclusions Women in both models of care considered the care as deficient in more than half of all areas. Variables that differed between the two models favoured group based antenatal care.

    Background: group-based antenatal care consists of six to nine two-hour sessions in which information is shared and discussed during the first hour and individual examinations are conducted during the second hour. Groups generally consist of six to eight pregnant women. Parent education is built into the programme, which originated in the United States and was introduced in Sweden at the beginning of the year of 2000. Objective: to investigate parents' experiences of group antenatal care in four different clinics in Sweden. Method: a qualitative study was conducted using content analysis five group interviews and eleven individual interviews with parents who experienced group-based antenatal care. An interview guide was used. Settings: the study was set in four antenatal clinics that had offered group-based antenatal care for at least one year. The clinics were located in three different areas of Sweden. Participants: the participants were women and their partners who had experienced group-based antenatal care during pregnancy. Other criteria for participation were mastery of the Swedish language and having followed the care programme. Findings: three themes emerged, 'The care-combining individual physical needs with preparation for parenthood, refers to the context, organisation, and content of care'. Group antenatal care with inbuilt parent education was appreciated, but respondents reported that they felt unprepared for the first few weeks after birth. Their medical needs (for physical assessment and screening) were, however, fulfilled. The theme, 'The group-a composed recipient of care', showed the participants role and experience. The role could be passive or active in groups or described as sharers. Groups helped parents normalise their symptoms. The theme, 'The midwife-a controlling professional', showed midwives are ignorant of gender issues but, for their medical knowledge, viewed as respectable professionals. Key conclusions: in the four clinics studied, group-based antenatal care appeared to meet parents' needs for physical assessment and screening. Parents identified that the groups helped them prepare for birth but not for parenthood. The group model created a forum for sharing experiences and helped participants to normalise their pregnancy symptoms. Implications for practise: the midwife's role in facilitating group-based antenatal care demands new pedagogical strategies and approaches.

    Abstract [en]

    Background: Higher levels of parental stress have long-term effects on children's health and could lead to dysfunction in the parent–child interaction. Different background factors can be predictors of high parental stress. Aim: The aim of this study was to examine parental stress among Swedish women and identify different factors linked to women's parental stress. Method: About 702 women were recruited to a clinical study and followed up six months after birth. Data were collected by two questionnaires, and 279 women completed the Swedish Parental Stress Questionnaire (SPSQ). Findings: Less than very good mental health and depressive symptoms after birth were strongly associated with parental stress, and the strongest association was found between post-partum depressive symptoms and high levels of stress in the subscale Incompetence. Multiparity was associated with high stress in two subscales, and lower level of education was a protective factor for stress in nearly all subscales. Conclusions: Depressive symptoms and perceived poor mental health post-partum are the most important factors related to high parental stress. The results point to the importance of identifying and supporting mothers with depressive symptoms, since these women have both mental illness and increased stress.

    Abstract [en]

    The aim of this study was to describe women's experience of living with a colostomy after rectal cancer surgery. Interviews with five women about their experiences were subjected to thematic content analysis. The findings showed that receiving a cancer diagnosis gave rise to thoughts about life and death. For the women to feel comfortable, the information and health-care measures need to focus on supporting them through the entire process, also when the treatment is completed. After the surgery, the women adjusted to living with colostomy and carried on as before the cancer diagnosis, but they constantly worried about leakage or flatulence. The women were happy to have survived the cancer and this realization helped them to accept and have a good life with colostomy. In conclusion, women with colostomy because of rectal surgery need specific rehabilitation and nursing care that focuses on adjustment to temporary or permanent changes in life. © 2010 Blackwell Publishing Asia Pty Ltd.

    Bakgrund: Den vanligaste formen av självskadebeteende kallas för ytligt självskadande. Det innebär att personen skär eller rispar sig själv med ett vasst föremål. Sjuksköterskor och annan vårdpersonal bör uppmanas att tänka över sina egna värderingar och sedan införliva det i arbetet med personer som skadar sig själva. Syfte: Syftet var att belysa sjuksköterskans upplevelser och attityder i omvårdnaden av personer med självskadebeteende. Metod: En litteraturstudie utfördes där 13 vetenskapliga artiklar inkluderades. Artiklarna granskades och bearbetades och sammanställdes till ett resultat. Resultat: Resultatet presenterades i två huvudkategorier och dessa var; upplevelser och attityder hos sjuksköterskan och behov av stöd. Sjuksköterskor bar på många känslor, så som frustration, rädsla, ångest och stress. Utbildning gav sjuksköterskorna nya färdigheter i omvårdnaden av personer med självskadebeteende. Diskussion: Den mest genomgripande känslan hos sjuksköterskorna i arbetet med personer som skadar sig själva var frustration. Den känslomässiga motviljan hos sjuksköterskorna gentemot dessa personer reducerades efter utbildningen. Slutsats: Brist på kunskap hos sjuksköterskorna kunde leda till en försämrad förståelse av självskadebeteende som i sin tur kunde leda till ett sämre bemötande. Behoven som sjuksköterskorna hade i omvårdnaden av personer med självskadebeteende var utbildning och en god arbetsmiljö.

    Bakgrund: Bröstcancer är idag en av de vanligaste cancerformerna och drabbar kvinnor världen över. Behandlingen innefattar främst kirurgi, och det kan bli aktuellt att avlägsna hela bröstkörteln, en så kallad mastektomi. Mastektomin kan ge fysiska och psykiska påfrestningar hos kvinnan, där sjuksköterskan har en betydelsefull roll gällande information och stöd till kvinnorna. Syfte: Att belysa upplevelsen av genomgången mastektomi hos kvinnor med bröstcancer. Metod: En litteraturöversikt baserad på åtta kvalitativa och sex kvantitativa studier som hämtats från databaserna Cinahl, PubMed och PsycINFO. Resultat: En genomgången mastektomi bidrog till en förändrad kroppsbild, som resulterade i en påverkad identitet, kvinnlighet och sexualitet. Fysiska och psykiska symtom uppkom, och stödet från familj, vänner och vårdpersonal hjälpte kvinnan att hantera mastektomin, samt i vissa fall att acceptera sin nya kropp. Diskussion: Eftersom kvinnan upplevde stora utseendemässiga följder och behovet av information och stöd var stort sågs sjuksköterskan som en viktig resurs i hanterandet av mastektomin. Sjuksköterskan blev därför en avgörande faktor för kvinnans upplevelse, vilket är att beakta i den kliniska praktiken. Slutsats: Litteraturöversikten ger en djupare förståelse för upplevelsen av mastektomi hos kvinnor, vilket är betydelsefullt för omvårdnadsarbetet, så att kvinnan kan få en mer individanpassad vård och ett ökat emotionellt stöd.

    Bakgrund: En tredjedel av Sveriges befolkning har återkommande eller långvarig smärta. Sedan 1980-talet har smärttillstånd blivit vanligare i landet. Vårdpersonal underskattar och negligerar ofta patienters smärta. Syfte: Att belysa hur personer med långvarig smärta upplever och hanterar sin situation. Metod: Litteraturöversikt med 12 kvalitativa vetenskapliga artiklar. Resultat: Långvarig smärta förändrar livet för de drabbade på många olika plan. Hur smärta hanteras är individuellt. Att lyssna på kroppen är en förutsättning för att kunna hantera smärta. De drabbade känner sig misstrodda och upplever sig sakna stöd från omvigning samt sjukvård. Diskussion: Flera svenska och internationella studier visar att personer med långvarig smärta känner sig misstrodda. Smärta är subjektiv och individuell. Alla patienter ska också mötas individuellt och vården ska anpassas efter denne. Slutsats: Utbildning inom smärta behövs för att höja kompetensen och förändra attityder hos vårdpersonal.

    Bakgrund: Kraven inom hälso- och sjukvården har ökat och sjuksköterskor måste ha kunskap inom många områden och samtidigt bedriva en säker vård. Arbetsmiljön är i många lägen bristfällig och har betydelse för arbetsrelaterad stress. Arbetsrelaterad stress uppstår när det upplevs att det inte finns möjlighet att lösa arbetsuppgifter på grund av otillräckliga kunskaper eller resurser. Syfte: Syftet var att belysa orsaker till arbetsrelaterad stress och hur sådan stress påverkar sjuksköterskors arbete och privatliv. Metod: Detta arbete utformades som en litteraturöversikt för att ge en övergripande bild av forskningsläget. Databassökningar gjordes och artiklar granskades, analyserades och ett resultat skapades som inkluderade 15 vetenskapliga artiklar. Resultat: I resultatet framkom fem huvudteman: “Arbetsmiljö och krav som orsak“, “Arbetslaget som orsak”, “Fysiska och psykiska effekter som drabbar sjuksköterskan”, “Påverkan på patientsäkerheten” och “ Behov av kunskap och kompetensutveckling”. Diskussion: I sjuksköterskors arbetsmiljö förekom förhållanden som orsakade hög arbetsbelastning och stress. Detta ledde till att många sjuksköterskor övervägde att lämna yrket. Sjuksköterskor oroade sig för kvaliteten på vården på grund av den höga arbetsbelastningen. Slutsats: Flera faktorer påverkade sjuksköterskors upplevelse av arbetsrelaterad stress. Dessa faktorer kan leda till konsekvenser, inte bara för patientsäkerheten utan också för personalens egen hälsa.     Abstract [en]

    Background:

    Patients admitted to hospital need nutrition to recover. Enteral nutrition is probably preferable to parenteral nutrition should be initiated as early as possible. Enteral nutrition may be administered in various ways, it can be intermittent and include night rest, or given continuously to avoid abrupt reductions in blood sugar levels. An unwanted complication is loose stools of the patient. The question is whether the form of administration of enteral nutrition can affect the frequency of diarrhea.

    Aim:

    With the help of a retrospective registry study comparing number of loose stools per day between two different forms of administration of enteral nutrition: intermittent with night sleep compared with continuous supply throughout the day.

    Method:

    A quantitative retrospective registry study has been carried out on a total of 50 ICU patients with traumatic head injury at Norrlands University Hospital. Records from 2007 to 2012 were used in the study.

    Results:

    No significant difference was detected between the groups in the number of diarrhea per day, p = 0.5. However, this study demonstrated that patients receiving intermittent enteral nutrition was significantly greater amount of enteral nutrition (699 ± 249 ml) per day compared with patients who received their enteral nutrition continuously (505 ± 278 mL / day), p = 0008.

    Conclusion:

    The results from the study show that the number of loose stools do not seem to depend on the form of administration of enteral nutrition. Quantities of enteral nutrition differ statistically significantly significance of which it should be further studied to determine which form of administration is preferred.

    Introduktion: En dryg femtedel av alla som får hjärt-lungräddning överlever men neurologiska komplikationer förekommer. Ett EJ HLR-beslut innebär att på förhand besluta om att avstå från hjärt-lungräddning. Hos medicinskt sjuka fattas ofta detta beslut utan den berördes vetskap. Syfte: Att belysa medicinskt sjuka personers tankar kring och delaktighet i ett beslut om att avstå från eventuell hjärt-lungräddning. Metod: Litteraturstudie med resultat från 15 vetenskapliga artiklar. Resultat: Flertalet tillfrågade (deltagare) ville vara delaktiga i beslutsprocessen om sitt EJ HLR-beslut. Samtalet om återupplivning upplevdes inte jobbigt, men tidpunkten måste väljas med omsorg. Det fanns en tvetydighet i vem som skulle ta beslutet, läkaren, deltagaren eller de tillsammans med viss fördel åt deltagaren. Faktorer som påverkade var aktuell hälsa, livskvalitet och sociala relationer. Diskussion: Att få vara delaktig i sitt EJ HLR-beslut är att respektera den personliga autonomin. Läkaren rekommenderas informera och därefter låta patienten ta ett eget beslut. Sjuksköterskan spenderar mycket tid med patienten och har en viktig stödjande roll. Slutsats: Ämnet är etiskt laddat, men medicinskt sjuka vill vara delaktiga i ett beslut om EJ HLR. Sjuksköterskan kan introducera ämnet och vara ett stöd. Rutiner för EJ HLR-beslut är idag otydliga och kan förbättras. Vidare forskning om tidpunkten för samtalet behövs.

    Abstract [en]

    The need to deliver high-quality palliative homecare has been underscored in current professional guidelines and demands for efficacy, as well as in the preferences of patients and their families. Indeed, demographic changes and an increased risk of cancer among older people pose challenges to the home healthcare settings in Norway and the rest of the world. Yet, little is known about how older persons experience living with incurable cancer and how palliative home nursing care may increase their quality of life. Moreover, few studies have focused on rural contexts or explored such settings can influence the delivery and outcome of care. In rural contexts, long distances and limited health-related human resources challenge the provision of specialised services, and palliative patients living in rural municipalities most often receive services from district nurses without special training in oncological or palliative care. The aim of this thesis was thus to explore experiences with, and meanings of, rural home nursing care among older persons living with incurable cancer. The thesis consists of five studies, all with qualitative designs, and performed in rural municipalities.Study I was designed as a case study involving individual interviews and observations to explore how older persons diagnosed with incurable cancer and living alone have experienced daily life while commuting for policlinic palliative chemotherapy. By extension, the aim of Study II, conducted as a secondary analysis of material collected in study I, was to illuminate and interpret the meanings of the lived experiences of the participants in that study. By contrast, Study III used individual interviews to illuminate and interpret the meaning of the lived experience of older persons with incurable cancer, yet who have received home nursing care. Meanwhile, Study IV entailed individual interviews with nurses working in home nursing care. Its aim was to illuminate and interpret the meaning of nurses' lived experiences among severely ill patients in their homes. Lastly, Study V involved individual interviews with bereaved family members; its aim was to explore their perceptions of suffering in older persons receiving palliative home nursing care during their final phase of life. The findings of Study I showed that older persons interviewed hovered between hope and fear, experienced stressful commutes, and were constantly exhausted. Experiences with long, tiring taxi trips, of having few supportive people nearby, and of being offered hardly any local healthcare services made these persons highly vulnerable. Nevertheless, their demands were few, and they rarely complained. The findings of Study II showed a complex, yet comprehensive situation in which physical symptoms and emotions had become entangled. Four themes were found: enduring by keeping hope alive, becoming aware of being one one's own, living up to expectations of being a good patient, and being at risk of losing identity and value. Suffering related to care, or the lack therefore, was the most striking discovery, and the older persons seemed to endure by keeping hope alive. Besides hoping for survival, their hopes also included a desire to be recognised and treated with respect, though such often seemed to go unnoticed. The findings of Study III revealed three themes: being content with what one gets, falling into place, and losing one's place. The phrase picking up the pieces was found useful for summing up the meaning of one's lived experience. In that sense, the three themes referred to how the pieces symbolized the remaining parts of life or services available in their environment and how the older persons might see themselves as pieces in a puzzle. Participants exhibited strong place attachment, involving physical insideness, social insideness, and autobiographical insideness, which suggested that the rural context might provide and advantageous healthcare environment. Interestingly, nurses' personal engagement and willingness to be involved in caring relationship appeared to be more important than any special competence or technical skills. The findings of Study IV showed that patients' expressions left impressions that caused emotional waves in the nurses. Four themes were found: being open for the presence of the Other, being satisfied, being frustrated, and being ambivalent. Understanding and balancing this emotional dimension in care seemed to cause confusion and distress for the nurses, and realizing how their feelings might prompt either generosity or aloofness towards the patient was upsetting. Thatinterpretation suggested confusion regarding what it means to be a professional nurse.Lastly, the findings of Study V showed that bereaved family members perceived suffering related to the illness, to the care, and to the life of their ill relatives during their final phase of life. Well-being emerged as having significant, contrasting elements. Well-being related to other people, to the home, and to activity, in all of which the essence referred to a state of dwelling-mobility. The findings suggested that nurses in this context need to seek out patients' and their families members' experiences with comfort and with disturbance. Nursing and palliative care that become purely disease- and symptom-focused can end with all parties' giving up and divert attention from social and cultural factors that may contribute to well-being when cure is not the goal.The findings of this thesis could alter some current knowledge in the field. From a patient perspective, the rural context is not necessarily disadvantaged, care is not necessarily caring, and the alleviation of suffering and cultivation of wellbeing are not necessarily two sides of the same coin. Home nursing care is often seen as a precondition for staying at home. Working in patients' homes allows nurses to witness and become involved in each patient's unique situation. However, impressions of patients' expressions awake feelings in nurses that have the power to bring about caring actions. At the same time, the fear of being unprofessional or unable to deliver proper care bothers nurses and might threaten the closeness that patients desire. Home nursing is care organized in a system in which services are predefined, apportioned, and bound to specific procedures, and the holistic ideals of the palliative care philosophy are often not within the framework of the system in which district nurses work.

    Abstract [en]

    Rural home nursing care is a neglected area in the research of palliative care offered to older cancer patients. Because access to specialized services is hampered by long distances and fragmented infrastructure, palliative care is often provided through standard home nursing services and delivered by general district nurses. This study aimed to illuminate the lived experience and to interpret the meaning of receiving home nursing care when being old and living with advanced cancer in a rural area in Norway. Narrative interviews were conducted with nine older persons, and a phenomenological hermeneutic approach was used to interpret the meaning of the lived experience. The analysis revealed three themes, each with subthemes: being content with what one gets, falling into place, and losing one's place. The phrase picking up the pieces was found useful to sum up the meaning of this lived experience. The three respective themes refer to how the pieces symbolize the remaining parts of life or available services in their environment, and how the older persons may see themselves as pieces or bricks in a puzzle. A strong place attachment (physical insideness, social insideness, and autobiographical insideness) is demonstrated by the informants in this study and suggests that the rural context may provide an advantageous healthcare environment. Its potential to be a source of comfort, security, and identity concurs with cancer patients' strong desire for being seen as unique persons. The study shows that district nurses play an essential role in the provision of palliative care for older rural patients. However, the therapeutic value of being in one's familiar landscape seems to depend on how homecare nurses manage to locate it and use it in a more or less person-centred manner. Communication skills and attentiveness to psychosocial aspects of patient care stand out as important attributes for nursing in this context.

    Det fanns många studier avseende attityder och kunskaper gentemot ECT bland både läkare och patienter. Däremot fanns ett fåtal studier ur sjuksköterskor och sjuksköterskestudenters perspektiv gällande attityd, kunskap och omvårdnad vid ECT behandlingen. Syftet var att ur ett omvårdnadsperspektiv beskriva ECT behandlingen avseende attityd, kunskap och omvårdnadshandling. Studien genomfördes som en litteraturöversikt baserad på tio vetenskapliga artiklar, nio kvantitativa och en kvalitativ, funna genom litteratursökning i databaserna Academic Search Elite, Cinahl, CSA Psychology, PsycInfo, PubMed, SweMed+, Scopus och Web of Science. Resultatet visade motsägelsefulla fynd angående attityderna. Bland sjuksköterskor och sjuksköterskestudenter fanns både positiva och negativa attityder till behandlingen. Kunskaperna var övervägande goda hos de båda grupperna. Omvårdnadshandlingarna, stöd och information anpassades individuellt efter patienternas situation och visade brister som förbättrades genom utbildningar. Sjuksköterskestudenter och sjuksköterskor bör ha en professionell attityd oavsett vårdsituationer. Möjligheten till vidareutbildning var en viktig del för bibehållen kompetens och utveckling. Omvårdnad är vår profession och omvårdnadshandlingar får aldrig brista. Slutsatsen var att vidare forskning angående sambandet mellan de tre fokusområden bör bedrivas.

    Abstract [sv]

    Bakgrund: Abdominellt aortaaneurysm (AAA) är en sjukdom där aortan blir utvidgad och riskerar att rupturera. Två operationsmetoder finns mot AAA, dessa är endovaskulär aneurysmbehandling (EVAR) och öppen operation OR.Syfte: Studiens syfte var att undersöka samt belysa hur livskvaliteten blev påverkad bland patienter som genomgått en öppen operation gentemot endovaskulär behandling för AAA.Metod: Designen av studien var integrativ litteraturöversikt och databaserna PubMed och Cinahl användes för datainsamling. Sökord som använts var av relevans för studiens syfte och 15 artiklar kvalitetsgranskades och var sedan tillämpade i studiens resultat.Resultat: Genomgången operation med EVAR eller OR har en negativ påverkan på livskvalitet. Symtom postoperativt påverkade patienter på ett sätt som de inte var förberedda på. Återhämtningsperioden postoperativt var längre än förväntat och ledde till oro och ångest hos patienterna. Kommunikation mellan vårdpersonal och patienter visade sig vara bristfällig.Diskussion: God smärtlindring, ett gott näringstillstånd, tillräckligt med vila och sömn samt delaktighet i sin vård ansågs vara förutsättningar för optimalt läkande.Slutsats: Postoperativ livskvalitet efter genomgången AAA operation visade sig bli negativt påverkad av olika faktorer, exempelvis undernäring, postoperativ smärta och sömnpåverkan. Det var bristande informationsutbyte mellan patienter och vårdpersonal postoperativt. Vården bör bli mer patientcentrerad för att vården och livskvaliteten skall kunna förbättras för AAA patienter postoperativt.

    Background: Involved fathers are important for childrens´ health and development. By giving parent support the child health care can increase childrens´ chance to good health. Aim: To illuminate the difficulties and challenges that fathers of small children experience and how they describe their need of support from the child health care. Method: A qualitative method was used. Ten fathers were interviewed and data was analysed using content analysis. Result:  Becoming a father was a major event in life and a positive experience. It involved difficulties and challenges, especially being able to handle both work and family life and to be responsible of another individual. The majority had a positive image of the child health care. They had many opinions about it and how they would like it to be. The fathers experienced a need that the child health care controlled the child´s health, gave advice when the child was ill and mediated contacts with other parents. It was important to be able to reach the child health care when they needed as well as the nurse knowing the child. Conclusion: Fathers experienced difficulties managing work and family life. The fathers´ needs of support from the child health care was greatest when the child was ill and they would like them to provide opportunities to meet other fathers. Committed fathers are important for the child´s health, development and well- being. Therefore, it is important for the child health care to work for fathers´ participation in the health care of the child. A field for future research is therefore to find and evaluate forms of parent support that  attract fathers.
