    Abstract [sv]

    Prognosen för överlevnad efter en hjärttransplantation har successivt förbättras, under år 2008 genomgick 44 personer i Sverige operationen. Då hjärtat kan ses som beviset för liv och centrum för våra känslor kan en hjärttransplantation ha en dramatisk inverkan på patienters dagliga liv och hälsa. Syftet med denna litteraturstudie var att belysa upplevelsen i samband med en hjärttransplantation och hur patienter hanterade sin situation. Metoden som användes var litteraturstudie där 16 artiklar inkluderades, publicerade mellan år 1998 och 2008. Data analyserades utifrån en innehållsanalytisk metod. Resultatet visade att patienter som levde med en svår hjärtsjukdom upplevde maktlöshet, begräns­ningar och behov av stöd. Att hantera sin livssituation i samband med en hjärttransplantation innebar användning av olika copingstrategier. Trans­planta­tion­en gav patienterna nya möjligheter och en känsla av tillfredställelse med livet, vilket hjälpte dem att blicka framåt. I diskussionen framkom att det liv som en hjärttransplantation erbjöd övervägde de motgångar och hinder som upplevdes i samband med ett nytt hjärta. Konklusionen var att genom vidare studier om patienters upplevelser i samband med en hjärttransplantation kan ny kunskap erhållas och omvårdnadsarbetet utvecklas ytterligare.

    E-health and information- and communication technology (ICT) as support systems for older family caregivers in rural areas2014Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    The overall objective of the thesis was to investigate how older family caregivers in rural areas experienced participation in an e-health based caregiver support system. Participants were 95 caregivers allocated to intervention group (n=63) and control group (n=32). The thesis had a mixed method design and is based on four original articles (I-IV). Data was collected through web-camera interviews (I-III), telephone interviews (II,IV) and questionnaires (II,IV) after 1.5 years of using e-health support. Quantitative data was analyzed using comparative statistics, multiple linear regression and logistic regression. Qualitative data was analyzed with content analysis. Article I showed that the caregivers, despite lacking experience from using computers, were able to adopt the new technology, with help from support nurses and family. Caregivers felt e-health helped them to regain social inclusion. Article II had a comparative design, comparing e-health support with conventional caregiver support. Caregivers using e-health were more satisfied with their support and found it to be available, flexible and helping them enhance caregiver competence. Control group caregivers were less satisfied with support and experienced unmet needs. Article III showed that e-health can reduce caregivers’ feelings of isolation. Two themes were created, Expanding the concept of place and Developing networks. Article IV revealed that caregivers used e-health frequently, experienced benefits from using it and had become more independent. Support nurses were disappointed about the decrease in contact with caregivers, and also acknowledged a need for developing the professional nursing role while working with e-health. Conclusions drawn from the findings were that participants experienced benefits from e-health support and it helped them become more independent. Swedish municipalities are obliged to provide caregiver support, the findings from the study are valid to conclude that e-health support are as good as, or even more effective, than conventional support for older family caregivers in rural areas.

    Abstract [en]

    The aim of this intervention study was to illuminate the meaning of ICT-basedcaregiver support as experienced by older family carers living in vast ruralareas, caring for a spouse at home. In order to access the support serviceparticipants were provided with a computer and high speed Internet in theirhomes. Semi structured webcam-interviews were carried out with 31 familycarers. A strategy for webcam interviewing was developed in order to ensurequality and create a comfortable interview situation for the family carers.Interviews were analyzed using content analyses, resulting in the themes: Adoptingnew technology with help from others and Regaining social inclusion.The results indicate that ICT-based support can be valuable for older familycarers in rural areas as it contributes to improve quality in daily life in anumber of ways. In order to fully experience the benefits, family carers needto be frequent users of the provided support. Adequate training andencouragement from others were essential in motivating family carers to use thesupport service. Access to Internet and webcamera contributed to reducingloneliness and isolation, strengthening relationships with relatives living faraway and enabled access to services no longer available in the area. Use of theICT-service had a positive influence on the relationship between the oldercarer and adult grandchildren. It also contributed to carer competence andpromote feelings of regaining independence and a societal role.

    We conducted a pragmatic, mixed methods study comparing rural family caregivers receiving e-health caregiver support (n = 35) with a control group (n = 21) receiving conventional, non-e-health, caregiver support. After 18 months, the benefits of support were evaluated using the Care Effectiveness Scale (40-items exploring the domains of preparedness, enrichment and predictability). In all domains the e-health group scored significantly higher than the control group. The adjusted difference for overall benefits was 3.0 (P = 0.02) on the scale 0-10. In addition, semi structured interviews were conducted with a sub-sample of the caregivers. For the e-health group flexibility, availability and being able to individualise the support were essential factors. All caregivers in the control group found conventional support to be beneficial, but also stressed unmet needs related to the conventional support being standardised and non-flexible. The study suggests that providers of caregiver support should offer e-health support as an alternative to conventional caregiver support, as it can be more beneficial to family caregivers.

    ICT, information- and communication technologies, and e-health services are essential for meeting future care demands. Greater knowledge regarding the implementation of e-health services in long-term care for older people is needed. The purpose of the study was to explore older people’s use of e-health services and associations between frequency of use and perceived benefits. In the longitudinal comparative intervention study (n = 65), intervention group participants (n = 42) used an e-health service for 1.5 years. A control group (n = 23) used similar services provided in a traditional manner. Data was collected through questionnaires and analyzed using linear and logistic regressions. Although general use of the Internet was similar in both groups, the e-health group perceived significantly higher benefits. The component information- and education programs, developed specifically for the e-health service, had the highest association with benefits. Conclusion: e-health services targeted at supporting older people who care for a spouse at home can provide benefits which most likely will not be obtained without participation in an organized e-service. Care professionals play an essential role in encouraging spouse caregivers to become e-service users.     BACKGROUND: Many older spouse caregivers are tied to the home by their caring duties and feel isolated. The values of supporting older caregivers are well known. In rural areas with long distances and decline in essential services, attending caregiver support groups can be difficult. Using Internet-based services can provide an opportunity for rural caregivers to participate in caregiver support, regardless of geographical distances and without the need for physical presence.

    AIMS AND OBJECTIVES: This study aimed to explore how Internet-based caregiver support may influence the experience of isolation among older spouse caregivers in rural areas.

    DESIGN: An intervention study where 63 older rural caregivers received an Internet-based caregiver support service.

    METHOD: A qualitative interview study based on 31 interviews with open-ended questions, analysed using latent content analysis.

    FINDINGS: Two themes represent the findings from the study: Expanding the concept of place and Developing networks. Even though participants still spent their days in the house, they experienced that daily life was being spent in a variety of places, both physically, virtually and emotionally. The Internet-based support service provided them with a tool to reconnect with family and develop new friends.

    CONCLUSIONS: Internet-based caregiver support may reduce the experience of isolation for spouse caregivers in rural areas. Nurses played a crucial part in the development, by encouraging, educating and inspiring caregivers and supporting their independence.

    IMPLICATIONS FOR PRACTICE: Internet-based services ought to be an option for caregiver support in rural areas as it may reduce feelings of isolation for older spouse caregivers.

    BAKGRUND: I världen råder oroligheter, människor tvingas fly från sina hemland och under år 2007 var det 67 miljoner människor som flytt sitt land. Att tvingas fly från sitt hemland kan vara en tillräcklig orsak till att utveckla psykisk ohälsa. Dessa människor behöver stöd från hälso- och sjukvården. Omvårdnadsteoretikern Leininger beskriver transkulturell omvårdnad som att med respekt för varje individs kultur anpassa omvårdnaden. SYFTE: Att belysa stödfaktorer av betydelse för flyktingar med psykisk ohälsa. METOD: Litteraturstudie med 15 vetenskapligt granskade artiklar. RESULTAT: Analysen resulterade i sex kategorier som sammanfattningsvis visade att fysiskt-, psykiskt-, andligt-, kulturellt-, socialt- och samhällets stöd hade betydelse för flyktingar med psykisk ohälsa. Att vara medveten om och ha förståelse för kulturspecifika faktorers påverkan på hälsan var av stor vikt för omvårdnad av flyktingar. DISKUSSION/SLUTSATS: Inom hälso- och sjukvården behövs utbildning och insikt i tvärkulturella möten. Detta anses nödvändigt för att kunna tillgodose flyktingar med psykisk ohälsa och deras behov av stöd. Arbetar man enligt Madeleine Leiningers omvårdnadsteori kan man som sjuksköterska erhålla kunskap som underlättar i beslut och handlingar i omvårdnaden av patienter från andra kulturer.

    Abstract [en]

    BACKGROUND: The main goal of this randomized controlled single-blinded pilot study was to study whether, independent of weight loss, a Palaeolithic-type diet alters characteristics of the metabolic syndrome. Next we searched for outcome variables that might become favourably influenced by a Paleolithic-type diet and may provide new insights in the pathophysiological mechanisms underlying the metabolic syndrome. In addition, more information on feasibility and designing an innovative dietary research program on the basis of a Palaeolithic-type diet was obtained. METHODS: Thirty-four subjects, with at least two characteristics of the metabolic syndrome, were randomized to a two weeks Palaeolithic-type diet (n = 18) or an isoenergetic healthy reference diet, based on the guidelines of the Dutch Health Council (n = 14). Thirty-two subjects completed the study. Measures were taken to keep bodyweight stable. As primary outcomes oral glucose tolerance and characteristics of the metabolic syndrome (abdominal circumference, blood pressure, glucose, lipids) were measured. Secondary outcomes were intestinal permeability, inflammation and salivary cortisol. Data were collected at baseline and after the intervention. RESULTS: Subjects were 53.5 (SD9.7) year old men (n = 9) and women (n = 25) with mean BMI of 31.8 (SD5.7) kg/m2. The Palaeolithic-type diet resulted in lower systolic blood pressure (-9.1 mmHg; P = 0.015), diastolic blood pressure (-5.2 mmHg; P = 0.038), total cholesterol (-0.52 mmol/l; P = 0.037), triglycerides (-0.89 mmol/l; P = 0.001) and higher HDL-cholesterol (+0.15 mmol/l; P = 0.013), compared to reference. The number of characteristics of the metabolic syndrome decreased with 1.07 (P = 0.010) upon the Palaeolithic-type diet, compared to reference. Despite efforts to keep bodyweight stable, it decreased in the Palaeolithic group compared to reference (-1.32 kg; P = 0.012). However, favourable effects remained after post-hoc adjustments for this unintended weight loss. No changes were observed for intestinal permeability, inflammation and salivary cortisol. CONCLUSIONS: We conclude that consuming a Palaeolithic-type diet for two weeks improved several cardiovascular risk factors compared to a healthy reference diet in subjects with the metabolic syndrome.     Introduction: In Sweden, prospective fathers are encouraged and welcome to attend prenatal visits, and pregnant women assess their partners' involvement in prenatal care as very important. The aim of this study was to describe expectant fathers' experiences of and involvement in prenatal care in Sweden. Methods: Data were drawn from a 1-year cohort study of 827 Swedish-speaking fathers recruited during their partners' midpregnancy and followed up 2 months after childbirth. Results: The participants reported that the most important issues in prenatal care were the woman's physical and emotional well-being and the support she received from her midwife. However, care was identified as deficient in nearly all aspects of information, medical care, and fathers' involvement. "Excessive" care was also reported and related to how the father was treated by the midwife, mainly in terms of attention to his emotional well-being. Discussion: Although fathers prioritize the needs of their pregnant partners, it is important for caregivers to assess fathers' needs and incorporate a family-oriented approach to prenatal care.

    Abstract [sv]

    Diabetes typ 2 är en kronisk sjukdom som är en av Sveriges största folksjukdomar med cirka 300 000 drabbade personer, ett stort mörkertal bär utan vetskap på sjukdomen. Diabetes typ 2 ökar kraftigt bland befolkningen i såväl Sverige som i övriga länder. Syftet med denna systematiska litteraturstudie var att belysa faktorer som är av betydelse för följsamhet vid egenvård hos personer med diabetes typ 2. Denna litteraturstudie baseras på 14 vetenskapliga artiklar varav nio är kvantitativ och fem är kvalitativa publicerades mellan år 2003-2008. Artiklarna analyserades och resulterade i fyra kategorier:Relationer där stöd var den största faktorn, Livsperspektiv vilket innefattar kulturell bakgrund och förnekelse, Lidande med ilska och frustration över att drabbats av sjukdom och Kunskap som innefattade brist på förståelse och kunskap om sjukdomen och dess behandling. Genom kontinuerlig egenvårdsorienterad utbildning med individen i fokus och med stöd och förståelse från omgivningen gynnades patientens egenvård. Ökade kunskaper hos vårdpersonalen om hur människor med diabetes upplever sin livssituation kan bidra till utveckling och förbättring av

    omvårdnaden. Vårdgivarens uppgift blir att motivera och uppmuntra patientens till livsstilsförändring.

    Bakgrund: Triage förekommer på många akutmottagningar för att bedöma patientens aktuella tillstånd och prioriteringsnivå för läkarkontakt. För detta moment finns flera olika skalor att förhålla sig till. Det är av vikt att prioritera rätt för att upprätthålla patientsäkerhet. Syfte: Att belysa allmänsjuksköterskans erfarenheter vid triagering av patienter på akutmottagning. Metod: Litteraturöversikt genomfördes genom sökningar efter relevanta artiklar i medicinska databaser. Artiklarna analyserades enligt Carlsson & Eimans bedömningsmallar och sammanställdes därefter i resultatet. Resultat: Erfarenheterna hos sjuksköterskor som utför triagering är många. Flera faktorer påverkar bedömningen. Det är inte alltid sjuksköterskan kan påverka dessa faktorer. Diskussion: De olika kategorierna går mer eller mindre in i varandra. Osäkerhet upplevs på grund av tidspress och brist på adekvata riktlinjer vilket kan påverka bedömningen av patienten. Som följd av de olika upplevelserna kan sjuksköterskan känna sig otillräcklig gentemot sina arbetsuppgifter, vilket kan leda till samvetsstress. Slutsats: Resultatet i litteraturstudien tyder på att triagering är komplicerat. Utveckling inom området är önskvärt genom vidare forskning.

    Bakgrund: Skadlig alkoholkonsumtion är en av de fem största riskfaktorerna till för tidig död. En viktig del vid behandling av alkoholrelaterade problem är motivationen. Sjuksköterskan kan påverka motivationen och förväntas identifiera och ingripa med målet att skapa ett förändrat beteende hos patienten. Syfte: Belysa faktorer som påverkar sjuksköterskans inställning till omvårdnadsarbetet att motivera patienter med skadlig alkoholkonsumtion till förändrade alkoholvanor. Metod: Denna uppsats är en litteraturöversikt där 17 vetenskapliga artiklar från tre olika databaser inkluderades. Dessa artiklar lästes, granskades, sorterades in i huvud- och underkategorier som sedan sammanställdes i resultatdelen. Resultat: Resultatet presenterades under en huvudkategori; sjuksköterskans syn på faktorer som påverkar omvårdnadsarbetet och fyra underkategorier; Identifiering av alkoholproblem, att ingripa och ge intervention, erfarenheter och upplevelser, kunskap och utbildning. Diskussion: Det förekom negativa attityder hos sjuksköterskan, men de hade en betydelsefull roll i att hjälpa patienter med alkoholrelaterade problem. Brist på kunskap och osäkerhet skapade ett behov av utbildning. Slutsats: Även om sjuksköterskan såg ett ansvar att hjälpa dessa patienter så förekom det osäkerhet som tyder på ett behov av mer utbildning och kunskaper för att stärka sin kompetens för att ingripa och identifiera patienter med alkoholrelaterade problem.

    Bakgrund: Diabetes typ 2 är en sjukdom som innebär nedsatt insulinsekretion och insulinkänslighet vilket kräver livsstilsförändringar. Den fysiska aktiviteten påverkar kroppens celler och är en viktig del i diabetasbehandlingen.

    Syfte: Syftet med denna litteraturstudie var att belysa rådgivningens betydelse för motivation till fysisk aktivitet samt hur motion påverkar upplevelsen av livskvalité i samband med diabetes typ 2.

    Metod: En litteraturstudie gjordes och artiklarna som ingår i studien har publicerats mellan år 1993 och 2005 med sökorden counseling, diabetes mellitus non insulin, exercise,intervention, nurse´och quality of life. Av 20 granskade artikklar motsvarade 13 artiklar litteraturstudiens syfte.

    Resultat: Resultatet påvisade att rådgivning och stöd förbättrade patientens motivation till fysisk aktivitet och ökade känslan av välbefinnande.

    Slutsats: Kunskapen om rådgivningens betydelse leder till större motivation till fysisk aktivitet och ökad upplevelse av livskvalité. Dessa faktorer bör uppmärksammas i vården av personer med diabetes typ 2.

Det går att se att efter en hjärtinfarkt är de som drabbats inte trygga med sig själva, rädda, trötta, men de genomgår också förändringar efter hjärtinfarkten och tar hjälp av anhöriga och sjukvårdspersonal för att uppnå detta.Diskussion: Här konstateras det att krisförloppet för hjärtinfarkt även kan ses bland andra patientgrupper samt att vissa som drabbas även får ut positiva effekter av sitt liv efter hjärtinfarktenSlutsats: Utifrån resultatet kan författarna dra slutsatsen att hjärtinfarkt påverkar patienternas livsupplevelse och skapar både negativa och positiva emotionella reaktioner. Det är viktigt att sjukvårdspersonal har kunskap och en förståelse för patientupplevelser efter en traumatisk kris för att kunna främja och förebygga onödigt lidande.

    Background

    The profession of operating room nurse is now the oldest nursing specialty, but remains primarily a hidden profession because it is exercised behind closed doors. The professional role were described as complex and revolved around the patient, cooperation and the technical equipment. Patricia Benners theories on the nurses’ professional development were used as a theoretical basis in the study. Aim The study aimed to elucidate how the operating room nurse perceives the professional role. Method The study was conducted with a qualitative approach and ten semi-structured interviews were carried out with operating room nurses from two hospitals in Sweden in spring 2011. All interviews were recorded and transcribed verbatim. To analyze the collected data content analysis with an inductive approach was used. Result In the text analysis twelve subcategories and four categories were identified: the role as being responsible of nursing, the experience of what makes the operating room nurse safe in the professional role, the meaning of being a co-worker during the work on the operating room and the view on the own profession Quotes from the informants were presented in the text to clarify the result. Conclusion The study showed that the operating room nurse has a central role for nursing in the surgical ward and the patient. The operating room nurse presence and expertise in operations was irreplaceable, but always dependent of the team around him/her.

    Bakgrund:

    Astma är en sjukdom i luftvägarna av kronisk inflammatorisk art och enligt WHO

    är det ca 235 miljoner människor över hela världen som lider av astma. Astma kan

    förekomma både hos barn och vuxna. Tack vare profylaktisk välfungerande behandling har

    behovet av sjukhusvård för personer med astma minskat, dock har prevalensen av bronkial

    astma ökat i Sverige och det förekommer fortfarande dödsfall vid väldigt svara

    astmaattacker.

    Syfte: Syftet med denna litteraturöversikt är att belysa faktorer av betydelse

    för egenvård hos personer med astma.

    Metod: Information och kunskap om astma som ämne

    inhämtades. Sökningar gjordes i databaserna PubMed och Cinahl. En systematisk

    granskning och analys av sammanlagt femton vetenskapliga artiklarna utfördes för att

    presentera resultatet.

    Resultat: Det finns manga faktorer som är av betydelse för egenvård av

    astma. Både förutsättningar hos personer med astma och hälso -och sjukvårdens roll kan

    påverka hur egenvarden hanteras av individer.

    Diskussion: Aspekter som bland annat att få

    en behandlingsplan, få råd om att göra ändringar i hemmet, skola eller arbetsmiljö är mindre

    förekommande inom vården, dessa saker bör ocksa inga i en patientutbildning för personer

    med astma.

    Slutsats: Genom att personer med astma får bättre kunskap om astma som

    sjukdom samt ökad förståelse om astmabehandling kan dessa individer lättare följa en

    behandlingsplan och bättre hantera sin egenvård.

    Bakgrund: Många patienter med schizofreni dör i förtid på grund av fysiska sjukdomar. Utebliven sjukdomsinsikt, stigmatisering, lägre deltagande vid preventiv screening, samt brister i vårdgivares attityder ansågs vara påverkande faktorer som kunde leda till försenade diagnoser och i slutändan en försämrad prognos. Syfte: Att belysa sjuksköterskors attityder och bemötande samt faktorer som påverkar dessa gentemot personer med schizofreni inom den somatiska vården. Metod: En litteraturstudie som grundades på tolv kvantitativa studier utifrån sökningar som gjordes i databaserna Cinahl, Pubmed och PsychInfo. Resultat: Sjuksköterskorna hade överlag en mer positiv än negativ syn på personer med schizofreni, men hade även fördomar och ett behov av socialt avstånd till dessa. Sjuksköterskors kunskaper om patientgruppen var låg. Diskussion: Högre utbildning och personlig kontakt med patientgruppen kunde ha ett samband med positiva attityder, medan mer kunskap inte syntes ha samma effekt. Slutsats: Sjuksköterskor behöver mer utbildning och en större medvetenhet om de egna attityderna. Det finns även ett behov av fler studier inom området.

    Abstract [en]

    Medicinal treatment of vasospastic Raynaud phenomenon is limited to primarily vasodilator medicines.

    OBJECTIVE:

    To explore the possible beneficial effects and tolerability of 120 mg two times a day of Ginkgo Biloba special extract EGb 761 in patients suffering from Raynaud disease (RD) (primary Raynaud phenomenon).

    METHODS:

    In a placebo-controlled, double-blind, pilot study, 41 patients with RD were randomized to either the active treatment group (EGb 761, n = 21) or placebo group for 10 weeks, after an initial 2-week run-in phase. The primary efficacy variables were self-reported changes of the frequency, duration, and severity of vasospastic attacks between the placebo-controlled run-in phase and the end of the study.

    RESULTS:

    Most of the patients were female, and both groups were perfectly matched with respect to demographic characteristics. The frequency of daily attacks reduced from 3.6 ± 2.3 to 2.4 ± 2.6 (-33%) in the EGb 761 group and from 2.9 ± 2.0 to 2.0 ± 1.8 (-31%) in the placebo group with no significant difference according to the ordinary least squares test (P = 0.3564). Furthermore, no significant differences were found with respect to the duration and severity of vasospastic attacks between the EGb 761 and placebo groups (P = 0.4392 and P = 0.7187, respectively). In all, 17 adverse events (AEs) were reported, 6 AEs from 5 patients in the EGb 761 group and 11 AEs from 8 patients in the placebo group. Serious AEs did not occur.

    CONCLUSION:

    EGb 761 treatment showed an excellent safety profile in patients with RD but could not demonstrate a statistically significant reduction in clinically relevant symptoms compared with placebo.

    Abstract [en]

    Introduction: As a result of unaesthetic and surgical development, more complex procedures are performed under local or regional anaesthesia, which allows for a patient to be conscious throughout the intraoperative phase.

    Aim: The aim of this thesis was to gain a deeper understanding of the intraoperative nursing care when the patient remains conscious during surgery. The thesis comprises two studies [I-II] and has, depending on the purposes, focused on the perspective of the operating theatre nurses [I] and the patients [II].

    Method: based on the aim, a qualitative approach was used for both studies[I, II]. Data were collected through focus group interviews [I] and individual interviews [I, II]. Collected data were analysed in accordance with grounded theory [I] and by using qualitative content analysis [II].

    Result: Study I showed that the main concern among the operating theatre nurses was to take the conscious patient into consideration by achieving and maintaining high ethical standards in the operating theatre throughout the process. In order to achieve high ethical standards, there were three aspects that the operating theatre nurses took into consideration: the patient, the team and the morale of the professional role. In order to achieve high morale, the nurses were vigilant and flexible and balanced between patient needs, team needs, and the needs associated with safe surgery. The ethical standards was described as a whole, with the goal of giving the patient a positive experience. Results in Study II showed that the patients felt that they were in a situation they could not control and they were attentive and reflective to make sense and feel trust in the situation. The overarching theme “to seek for trust in a sensitive and dependent situation”, illustrates how the patients tended to assess the events that occurred around them in such a way that it brought the feeling of being in safety in the first place. The four categories; to experience the OT setting, to meet (un)expected sensations, to perceive evidence of safety, and to use individual strategies describes their experiences of what could affect the trust and how they handled this experiences. Main findings concerned the intraoperative needs and the handling and balancing between them.

    Conclusion: The operating theatre nurse´s intraoperative nursing care contributes to positive experiences for the patient who are conscious during surgery. The operating theatre nurse´s ability to interpret and to act on different intraoperative needs and the patient´s personality and ability to handle the situation are of great importance in the experience of the intraoperative phase.

    Länge ansågs det både bland allmänhet och bland läkare, att då hjärtat slutar att slå, var döden oundviklig. Det faktum att hjärtat kan startas på nytt och att människor kan överleva har skapat etiska dilemman. Det optimala är om återupplivningsförsök endast påbörjas på patienter med potential för långt överlevande, vilket kan vara svårt att avgöra i ett tidigt stadium. Vid hjärtstillestånd krävs beslut inom sekunder. Den diskussion som förs av bland andra Svenska Rådet för hjärtlungräddning handlar dels om hur man kan öka chanserna till överlevnad vid hjärtstillestånd men även om att försöka minimera omotiverade återupplivningsförsök. Syftet med studien var att bland ambulanspersonal i Sverige kartlägga uppfattningar i frågor som rör beslut om att påbörja återupplivningsförsök. Den empiriska studien byggde på en enkätundersökning med tvärsnittsdesign. Resultatet visade att ambulanspersonalen som helhet anser att vissa faktorer, vilka är förknippade med låg överlevnadschans, ska ha betydelse vid beslut om att påbörja återupplivningsförsök. Skillnader förelåg dock beroende på yrkeskategori (ambulanssjukvårdare/sjuksköterska) eller antal år inom yrket. Hur personalen faktisk agerade har inte kartlagts. I diskussionen konstaterades en tydlig variation i ambulanspersonalens uppfattningar. Om denna variation speglade det faktiska agerandet vid omhändertagande av patienter med hjärtstopp innebar detta ett betydande godtycke.

Det krävs kunskap för att sjuksköterskan ska kunna motivera ochstärka patientens egenmakt(empowerment) till att förändra livsstilen genom exempelvis kostoch fysisk aktivitet.SYFTE:Syftet med denna litteraturöversikt är att belysa hur sjuksköterskan med stöd av motiverandesamtal och hälso coachande samtal kan motivera patienter med DM2 till livsstilsförändring.METOD:Litteraturöversikten grundas i artiklar tagna från databaserna PubMed och Cinahl. De valdaartiklarnas innehåll har noggrant granskats och analyserats av båda författarna för att finnaspecifik data som besvarar syftet.RESULTAT:Resultatet visade på positiva effekter och tveksamma effekter av motiverande samtal ochhälso-coachande samtal. Resultatet visade även på svagheter och styrkor i sjuksköterskansmotiveringsarbete.DISKUSSION: Sjuksköterskor behöver mera utbildning av motiverande samtal ochhälso-coachande samtal för att kunna få mer positiva effekter utav motivationsarbetet tillpatienterna.SLUTSATS: Resultatet påvisar positiva effekter och tveksamma effekter av motiverande samtaloch hälso-coachande samtal. Utbildning för sjuksköterskor i MI, hälso-coachande samtal ochempowerment behöver förbättras för att kunna öka framgången i sjuksköterskans arbete attmotivera patienter med DM2 till livsstilsförändringar.

    Abstract [en]

    Nurses working with cancer patients in end of life care need to be prepared to encounter patients' psychosocial and spiritual distress. Aim: The aim of this study was to describe nurses' experiences of existential situations when caring for patients severely affected by cancer. Methods and sample: Nurses (registered and enrolled) from three urban in-patient hospices, an oncology clinic and a surgery clinic and a palliative homecare team were, prior to the start of a training program, invited to write down their experiences of a critical incident (CI), in which existential issues were featured. Results: Eighty-eight CIs were written by 83 nurses. The CIs were analyzed with qualitative content analysis. Two main themes were found: Encounters with existential pain experiences, which concerned facing death and facing losses; and Encountering experiences of hope, which concerned balancing honesty, and desire to live. Conclusions: This study points out that health care professionals need to be aware of patients' feelings of abandonment in exposed situations such as patients' feelings of existential loneliness. That there are some patients that express a desire to die and this makes the nurses feel uncomfortable and difficult to confront these occurrences and its therefore important to listen to patients' stories, regardless of care organization, in order to gain access to patients' inner existential needs.

    Objective: The objective of this study was to explore health care staff 's opinions about what existential issues are important to patients with cancer and staff 's responsibility when existential issues are raised by patients. Method: Four focus group interviews were conducted with health care staff (N = 23) at an in-patient hospice, on an oncology ward, on a surgical ward, and with a palliative home health care team. The focus group interviews focused on two questions, first, about health care staff 's opinions about patients' important existential questions and, second, about health care staff 's responsibility when existential issues are raised by the patient. The interviews were taperecorded, transcribed verbatim, and analyzed by qualitative content analysis into subcategories and categories. Results: Four categories and 11 subcategories emerged from the first question. The first category, "life and death," was based on joy of living and thoughts of dying. The second category "meaning," consisted of acceptance, reevaluation, hope, and faith. The third category, "freedom of choice," consisted of responsibility and integrity, and the fourth and last category, "relationships and solitude," consisted of alleviation, dependency, and loss. One category emerged from the second question about the health care staff 's responsibility, "to achieve an encounter," which was based on the subcategories time and space, attitudes, and invitation and confirmation. Significance of results: One strength of this study was that the findings were fairly congruent in different settings and in different geographical areas. Health care staff were aware of the importance of existential issues to patients. The existential issues, mentioned by health care staff, are similar to findings from studies conducted among patients, which is another strength of the present study. Health care staff are also confident about how to act when these issues are raised by the patients. The challenge for the future is to implement the findings from this study among health care staff in different settings.

    The purpose of the study was twofold: to examine what type of daily stressful events post-menopausal woman with breast cancer experience during adjuvant chemotherapy and how bothersome these are and to identify coping strategies used by these women used to manage such stressful events. The patient group comprised 75 consecutively invited women (≥55 years of age) at two university hospitals and one county hospital in Sweden. The Daily Coping Assessment was used to collect data over time. Data were analysed both qualitatively and quantitatively. Six categories of stressful events were identified: `nausea and vomiting', `fatigue', `other symptoms', `isolation and alienation', `fear of the unknown' and `being controlled by the treatment'. The first three categories were subsumed under the domain physical problems and the latter three under psychosocial problems. Almost 30% of the diary entries recorded no stressful event. Physical problems were three times as frequent as psychosocial problems. `Nausea/vomiting' was the most frequently observed stressful event (21.6%). `Isolation and alienation' and `fear of the unknown' were less frequent, but when they occurred they were rated as the most distressing. Several coping strategies were used to manage each stressful event. The most common strategies were acceptance, relaxation and distraction. Religion was rarely used as a coping strategy.

    The purpose of the present study was twofold: first, to describe changes of Health-Related Quality of Life (HRQoL) during the adjuvant treatment among postmenopausal women with breast cancer; second, in the same population to identify the best predictors of Overall Quality of Life (QoL) after treatment, from perceived functioning, symptoms, emotional distress and clinical/demographic variables measured at baseline. The study group was 150 women (>= 55 years of age) scheduled for adjuvant chemotherapy (CT, n=75) or radiotherapy (RT, n=75). They were examined before (baseline), during and after completing the treatment. Data about QoL, perceived functioning, symptoms and emotional distress were collected with the European Organisation for Research and Treatment of Cancer (EORTC)-QLQ-C30, BR23 and Hospital Anxiety and Depression Scale (HADS) questionnaires. The general finding was that the adjuvant treatments were associated with decrease in overall QoL, physical and role functioning, anxiety and body image, as well as with increase in fatigue, dyspnoea, pain, nausea/vomiting, constipation and systemic therapy side effects measured over time. For women receiving CT, better emotional functioning and less pain at baseline predicted better overall QoL at the end of the treatment. For women receiving RT, better physical and emotional functioning, less breast symptoms and lower tumour stage at baseline predicted better overall QoL at the end of the treatment.

    Ten women and eight men who were caring for their demented husbands or wives participated in a closed-group counselling programme, developed and carried out by two district nurses at a local health centre. There were seven or eight participants in each group, which met 13-14 times over a period of 8months.This study is based on semistructured interviews about the participants' situation just before entering the counselling group, the counselling programme itself, and their situation after the end of the programme. Their situations before the programmes were described as an exhausting, chaotic prison but after the programme they could cope with their situation and plan and manage their daily life. None of the participants needed further organised counselling; engagement in the local dementia association was sufficient for them. The counselling nurses' experience in and about caring for demented patients, their tactful authority, the closed groups and the long duration of the programme were judged to be crucial for the successful outcome of the programme.

    Abstract

     

    Background: Depression is one of the most common causes of mental affliction and adolescents are largely affected. Depression leads to anxiety, isolation and often self-harming behavior. Adolescents seldom seek help. More research on adolescents is needed. Aim: To elucidate adolescent’s experiences of depression from a family perspective. Method: Systematic review containing 15 peer-reviewed articles published between 2001-2011 that have been analyzed and reviewed. Results: The adolescents did not want to talk about their feelings, of fear that others would not understand. They became isolated and had no will to do homework or partake in social relationships. When contact had been taken with family or friends, the adolescents tended to seek professional care. When the nursing worked, feelings of great relief occurred, but fear of relapse remained. Discussion: It was important that the care showed that they understood the disease and it was essential that the adolescents had someone to talk to that they could trust. Conclusion: Depressed adolescents experienced themselves as being different, seldom contacted with the healthcare and worried about how others would react to their feelings. The family was an important part in the nursing process. Also understanding and respect must be included to remedy the problem.
