    E-health and information- and communication technology (ICT) as support systems for older family caregivers in rural areas2014Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    The overall objective of the thesis was to investigate how older family caregivers in rural areas experienced participation in an e-health based caregiver support system. Participants were 95 caregivers allocated to intervention group (n=63) and control group (n=32). The thesis had a mixed method design and is based on four original articles (I-IV). Data was collected through web-camera interviews (I-III), telephone interviews (II,IV) and questionnaires (II,IV) after 1.5 years of using e-health support. Quantitative data was analyzed using comparative statistics, multiple linear regression and logistic regression. Qualitative data was analyzed with content analysis. Article I showed that the caregivers, despite lacking experience from using computers, were able to adopt the new technology, with help from support nurses and family. Caregivers felt e-health helped them to regain social inclusion. Article II had a comparative design, comparing e-health support with conventional caregiver support. Caregivers using e-health were more satisfied with their support and found it to be available, flexible and helping them enhance caregiver competence. Control group caregivers were less satisfied with support and experienced unmet needs. Article III showed that e-health can reduce caregivers’ feelings of isolation. Two themes were created, Expanding the concept of place and Developing networks. Article IV revealed that caregivers used e-health frequently, experienced benefits from using it and had become more independent. Support nurses were disappointed about the decrease in contact with caregivers, and also acknowledged a need for developing the professional nursing role while working with e-health. Conclusions drawn from the findings were that participants experienced benefits from e-health support and it helped them become more independent. Swedish municipalities are obliged to provide caregiver support, the findings from the study are valid to conclude that e-health support are as good as, or even more effective, than conventional support for older family caregivers in rural areas.

    Abstract [en]

    The aim of this intervention study was to illuminate the meaning of ICT-basedcaregiver support as experienced by older family carers living in vast ruralareas, caring for a spouse at home. In order to access the support serviceparticipants were provided with a computer and high speed Internet in theirhomes. Semi structured webcam-interviews were carried out with 31 familycarers. A strategy for webcam interviewing was developed in order to ensurequality and create a comfortable interview situation for the family carers.Interviews were analyzed using content analyses, resulting in the themes: Adoptingnew technology with help from others and Regaining social inclusion.The results indicate that ICT-based support can be valuable for older familycarers in rural areas as it contributes to improve quality in daily life in anumber of ways. In order to fully experience the benefits, family carers needto be frequent users of the provided support. Adequate training andencouragement from others were essential in motivating family carers to use thesupport service. Access to Internet and webcamera contributed to reducingloneliness and isolation, strengthening relationships with relatives living faraway and enabled access to services no longer available in the area. Use of theICT-service had a positive influence on the relationship between the oldercarer and adult grandchildren. It also contributed to carer competence andpromote feelings of regaining independence and a societal role.

    We conducted a pragmatic, mixed methods study comparing rural family caregivers receiving e-health caregiver support (n = 35) with a control group (n = 21) receiving conventional, non-e-health, caregiver support. After 18 months, the benefits of support were evaluated using the Care Effectiveness Scale (40-items exploring the domains of preparedness, enrichment and predictability). In all domains the e-health group scored significantly higher than the control group. The adjusted difference for overall benefits was 3.0 (P = 0.02) on the scale 0-10. In addition, semi structured interviews were conducted with a sub-sample of the caregivers. For the e-health group flexibility, availability and being able to individualise the support were essential factors. All caregivers in the control group found conventional support to be beneficial, but also stressed unmet needs related to the conventional support being standardised and non-flexible. The study suggests that providers of caregiver support should offer e-health support as an alternative to conventional caregiver support, as it can be more beneficial to family caregivers.

    ICT, information- and communication technologies, and e-health services are essential for meeting future care demands. Greater knowledge regarding the implementation of e-health services in long-term care for older people is needed. The purpose of the study was to explore older people’s use of e-health services and associations between frequency of use and perceived benefits. In the longitudinal comparative intervention study (n = 65), intervention group participants (n = 42) used an e-health service for 1.5 years. A control group (n = 23) used similar services provided in a traditional manner. Data was collected through questionnaires and analyzed using linear and logistic regressions. Although general use of the Internet was similar in both groups, the e-health group perceived significantly higher benefits. The component information- and education programs, developed specifically for the e-health service, had the highest association with benefits. Conclusion: e-health services targeted at supporting older people who care for a spouse at home can provide benefits which most likely will not be obtained without participation in an organized e-service. Care professionals play an essential role in encouraging spouse caregivers to become e-service users.     BACKGROUND: Many older spouse caregivers are tied to the home by their caring duties and feel isolated. The values of supporting older caregivers are well known. In rural areas with long distances and decline in essential services, attending caregiver support groups can be difficult. Using Internet-based services can provide an opportunity for rural caregivers to participate in caregiver support, regardless of geographical distances and without the need for physical presence.

    AIMS AND OBJECTIVES: This study aimed to explore how Internet-based caregiver support may influence the experience of isolation among older spouse caregivers in rural areas.

    DESIGN: An intervention study where 63 older rural caregivers received an Internet-based caregiver support service.

    METHOD: A qualitative interview study based on 31 interviews with open-ended questions, analysed using latent content analysis.

    FINDINGS: Two themes represent the findings from the study: Expanding the concept of place and Developing networks. Even though participants still spent their days in the house, they experienced that daily life was being spent in a variety of places, both physically, virtually and emotionally. The Internet-based support service provided them with a tool to reconnect with family and develop new friends.

    CONCLUSIONS: Internet-based caregiver support may reduce the experience of isolation for spouse caregivers in rural areas. Nurses played a crucial part in the development, by encouraging, educating and inspiring caregivers and supporting their independence.

    IMPLICATIONS FOR PRACTICE: Internet-based services ought to be an option for caregiver support in rural areas as it may reduce feelings of isolation for older spouse caregivers.

    En samordnad kvalitetsmätning har genomförts på samtliga 107 särskilda boenden för äldre i Västernorrlands län. Studien belyser själva arbetsprocessen, medarbetares upplevelser och ger förslag på förbättringar inför framtida mätningar. Resultatet visar att kvaliteten generellt är god framför allt när det gäller kunskap om och tillämpning av rutiner och regelvek - men det finns förbättringsområden inom framför allt omvårdnadsområdet.

    En vårdhund är en hund som utbildats till arbete inom vård, skola eller omsorg. På ett särskilt boende för äldre i Härnösands kommun har arbets-terapeuten använt vårdhund som redskap vid rehabilitering. Två olika grupper fick träna med vårdhunden: personer som var i fysiskt god form men inte ville delta i boendets aktiviteter på grund av exempelvis demens, ångest, oro eller aggression samt personer som efter träning på traditionellt vis inte hade uppnått de mål som de borde ha uppnått efter träningsperioden. Totalt tränade 27 personer med vårdhundteamet. Utvärdering av träningen gjordes separat för varje grupp med fokus på livskvalitet respektive ADL-förmåga. De instrument som användes var QUALID och ADL-taxonomin. Samtliga av dem som tränade med hunden hade efter avslutad tränings-period ökat sin grad av livskvalitet eller förbättrat sin ADL-förmåga. Hunden medförde ökad glädje och meningsfullhet i vardagen. Hunden bi-drog också till att aggressiviteten hos en del personer minskade, och att det blev lugnare på avdelningen. I vissa fall kunde lugnande mediciner sättas ut efter träning med hunden.

    Flera undersökningar har visat att det finns brister i kvaliteten i äldres läkemedelsanvändning. Äldre behandlas ofta i onödan och får ibland läkemedel som inte passar ihop. I syfte att förbättra kvaliteten i läkemedelsanvändningen har Timrå Kommun under ett års tid prövat en rutin för läkemedelsgenomgång enligt distansmodellen. Målet med projektet var att på ett kvalitetssäkrat sätt kunna utvärdera läkemedelsanvändningen, och om möjligt minska antalet läkemedel per vårdtagare, för att uppnå optimal hälsa hos äldre personer i särskilt boende.

    Med utgångspunkt i ett salutogent synsätt har personal på Älandsgårdeni Härnösands kommun utvecklat ett nytt arbetssätt för sitt dagligaarbete. Syftet med utvecklingen var att ge personer med demenssjukdomgoda förutsättningar att behålla sin naturliga dygnsrytm samt attskapa en harmonisk miljö där de äldre ges möjlighet att behålla sitt”jag” och därmed känna att de ingår i ett sammanhang.Med hjälp av olika verktyg, bland annat planeringstid, en tydligt uttaladvårdfilosofi och förändring av personalens arbetstider har manuppnått flera positiva förändringar, både för de äldre som bor påÄlandsgården, deras anhöriga och personalen.

    En hemtjänstgrupp har granskat sin verksamhet och dess förmåga att leverera utifrån antagandet att syftet med hemtjänsten är att ge sina kunder det stöd de behöver för att klara sig själva och uppnå livskvalitet. Studien visade att hemtjänstorganisationen inte var särskilt bra på att leverera det som kunderna ansåg vara viktigt. I stället för att utgå från vad som var viktigt för kundernas stod organisationens behov ofta i centrum, vilket ledde till bristande kvalitet.Kunderna ansåg att kontinuitet, att det är samma personer som kommer hem till dem, var mycket viktigt. I genomsnitt besöktes varje kund av 26 olika personer varje månad. Den som hade sämst kontinuitet fick hjälp av 58 olika personer. I rapporten redovisas fakta om hemtjänstens leveransförmåga, en kartläggning av vad som orsakar bristerna samt idéer till förbättring.Författare:
