    Abstract [en]

    Objective: The objective of this study was to from a gender perspective, explore elementary school teacher' experiences of their work situation, and identify conditions that could be health risks. Participants: Eighteen female teachers who work in an elementary school in Northern Sweden. Method: Thematic interviews were conducted using an interview guide. The interviews were tape-recorded and transcribed verbatim. Qualitative content analysis was used to analyse the transcribed text and interpretations were made within gender theory. Results: Four categories emerged: "Squeezed between dream and reality", "Effort to keep up with demands", "We can make it together" and "The school needs men's qualities". The categories were linked together with the theme "A struggle for time to teach". The theme describes the conflict between the teachers' ambitions to teach and create a stimulating learning environment versus the increased need for behaviour control that took time from classroom work. Beside work at the school, the teachers carried a large burden of domestic work.Conclusions: Teachers' work includes both endless demands and great joy. Their work is structured within the schools gender system in which caring duties are subordinated despite a growing demand for behaviour control. Traditional gender roles affect their domestic work load.

    A short-term evaluation of vocational rehabilitation (VR) may give conclusions not automatically applicable over a longer term. The present study follows up alterations in work resumption or in social insurance benefits from the time of completed VR and during the following two years. AIM: The primary objective was to evaluate work resumption among previous sick-leavers granted vocational rehabilitation. The aim of the follow-up was to assess the stability of the outcome of VR over time and to analyse factors of importance for clients that remained at work. METHOD: A register investigation was based on 815 cases where the clients had taken part in vocational rehabilitation and were served by one of six local social insurance offices of a Swedish county. RESULTS: Of the clients studied, 52.4% had attained full working capacity The proportion had decreased to 37.4% two years later. One factor that differed between those who resumed work and those who returned to sick leave was the duration of the previous sick-leave period. Those who returned to work had had shorter sick leave, had jobs to return to and had received job training as a vocational rehabilitation measure. CONCLUSIONS: The clients with the best chances of being in work two years after completed vocational rehabilitation were those with short sickness absence, who had been selected for job training as a vocational rehabilitation, were aged 16-29 years and were employed in industry.

    This study is a prospective multicentre cohort study entitled Work and Health in the Processing and Engineering Industries, the AHA Study (AHA is the Swedish abbreviation for the study). Four large workplaces in Sweden participated during the years from 2000 to 2003. The present report has two objectives: (1) to present a comprehensive occupational health intervention programme and (2) to evaluate this programme with a focus on lifestyle (smoking and exercise), health related quality of life (HRQoL) and sick leave. Interventions were provided on an individual and group level, including evidence-based methods for four health/focus areas (individual level) and a group intervention based on a survey-feedback methodology. The analyses in this report were exclusively employed at an organizational level. The proportion of smokers decreased at three companies and the course of the HRQoL was advantageous at two of the companies as compared to a gainfully employed reference group. A significant decrease in sick leave was revealed at one company, whereas a break in an ascending sick-leave trend appeared at a second company as compared to their respective corporate groups. This comprehensive workplace intervention programme appears to have had positive effects on smoking habits, HRQoL and sick leave.

    Using a multilevel approach, this study explores how job demands and control are dependent on the organizational context in which work is performed. The data set consisted of 3,485 employees at 51 establishments divided into 141 sub-units and belonging to 10 parent organizations. Data were collected by means of a survey answered by the employees and structured interviews conducted with operative managers at participating establishments. The results showed that a significant proportion of the variance in job demands and control was attributed to the organization, and that the three organizational levels varied in terms of their impact on the two psychosocial dimensions. More specifically, job demands were mostly affected by the establishment level, while the sub-unit level seemed to be the most important for job control. It is concluded that in studies of working conditions, the organizational context should be considered.

    The objective was to compare the applicability of and results provided by the two measures of sickness absence used most often within the Swedish social insurance administration (that is, unadjusted sick-leave rate and adjusted sick-leave rate) and five measures suggested by epidemiological researchers. Data consisted of four cross-sectional data sets of registry sick-leave data covering four separate years (1997-2000) in three counties. In total 454,000 persons qualified for sickness insurance and aged 20-64 years were included. The two measures used within the social insurance administration and three of the five measures suggested by epidemiological researchers revealed sex-related dissimilarities in absence patterns that indicated that women had more sickness absence than men. However, in marked contrast to those results, two of the epidemiologically based measures (i.e., length of sickness absence and duration of sickness absence) instead showed highly comparable rates of sick leave for men and women, and such information is seldom obtained, albeit definitely of importance, when trying to make a correct assessment of sickness absence. The measure of sickness absence that is used influences the findings and should therefore be chosen with care. Complementing the measures used in the social insurance administration by five measures suggested by epidemiological researchers provided a more informative and comprehensive picture of sickness absence in a population. Further investigations into the effect of using different measures is needed, as well as international consensus on what to call different measures.

    The relation between empowerment and burnout was investigated using a two-wave and two-variable panel model. A questionnaire was completed on two occasions, one year apart, by 1,356 nurses. The paths in the model were estimated using regression analyses and the following results were obtained: (a) empowerment is a stable state; (b) higher levels of empowerment are related to lower levels of burnout, when empowerment and burnout are studied at the same point in time; (c) higher prior levels of empowerment can be associated with higher later levels of burnout, when empowerment and burnout are studied at different points in time; (d) for assistant nurses, but not for registered nurses, prior higher levels of burnout resulted in lower later levels of empowerment, indicating a reciprocal relation between the two concepts. The results imply that what is a protective factor from a cross-sectional perspective may be a risk factor from a longitudinal perspective.

    This study analysed self-assessed quality of life (QoL), using a QoL questionnaire (Hörnquist's QLcs) covering life spheres, somatic health, mental well-being, cognitive ability, social and family life, activity, financial situation, meaning in life and a global score "entire life", for young people aged 18-24 in a population-based cross-sectional study in northern Sweden. Of these, 651 were unemployed and 2802 were in work (employed, students and in military service). Results showed that the young unemployed exhibited poorer QoL than the young in work and the greatest difference was found regarding their financial situation. Young men rated somatic health and mental well-being higher than young women. However, QoL in other essential domains was rated higher by young women in work. Close friends and money reserve were important for all participants, no matter whether they were employed or not. The risk of being young and unemployed was greater if the person had a worse financial situation, shorter education, and fewer leisure activities with other people. Finally, it was concluded that while QoL is poorer when in unemployment - both for the young and those who are older (aged 25-64) - psychological well-being, in contrast to several previous studies, is even poorer for young people than for those who are older. This is worrying in a public health perspective and could have implications for unemployment policies for younger and less well-educated age groups.

    This study analysed self-assessed quality of life (QoL), using a QoL questionnaire (Hörnquist's QLcs) covering the life spheres, somatic health, mental well-being, cognitive ability, social and family life, activity, financial situation, meaning in life and a global score "entire life", in 487 unemployed subjects and 2917 employed subjects aged 25-64, in a population-based cross-sectional study in northern Sweden. In line with previous findings, results showed that unemployed people exhibited poorer QoL. Unemployed women scored higher in existential life domains than unemployed men did. Unemployed men were worst off in terms of general life situation. Employed respondents benefited in QoL by a university/college education, while unemployed respondents with a university/college education did not. Close social relations and money reserve were associated with higher QoL generally. It was concluded that further research is needed to differentiate various aspects of QoL and unemployment, and to compare with other samples.

    Many women require vocational rehabilitation before they can return to work. The objective with the present study was to describe female clients' situation during the rehabilitation process, as it became apparent during the rehabilitation meetings with the various actors involved. The clients' diagnoses varied, but the majority was affected with musculoskeletal disorders. The meetings were audio-taped and transcribed verbatim, after which they were analysed by qualitative content analysis. The following themes emerged: Adaptation to the rehabilitation group; client's health status; the workplace's significance for rehabilitation; and the client's decision-point. Conflict between health and work was immensely important for rehabilitation. Differences in attitude were apparent during the rehabilitation meetings, as some clients were passive and exercised less influence on the planning, than the other more active clients did.

    PURPOSE: The aim of the present study was to investigate differences in the effects on employment between clients whose rehabilitation was coordinated in systematic multiprofessional rehabilitation group meetings and clients whose rehabilitation was coordinated in the "conventional" way. METHOD: The study was based on a sample of 51 individuals who received systematic group meeting coordination. All individuals in the study group were unemployed before the rehabilitation intervention. Two different comparison groups were chosen: one at a local level and another at a national level. The groups were matched on an individual level based on records obtained from The National Labour Market Board (AMS) and The National Social Insurance Board (RFV). The data were analysed by an analysis of variance (ANOVA) for repeated measures for binary responses. RESULTS: 68.6% in the study group and 49% in both the local comparison group and national group had some form of employment 24 months after rehabilitation. The ANOVA analyses (in terms of odds ratio) found that when all measurement points (6, 12 and 24 months after the rehabilitation) were included in the calculations that there was twice as high a chance of becoming employed after having received rehabilitation services through the multiprofessional group than for both comparison groups. The majority of employment in all the groups was associated with some form of subsidy or sheltered employment.

    PURPOSE: The purpose of this research was to investigate the views of sick-listed employees' immediate superiors on co-operation in vocational rehabilitation before and during a systematic, multi-professional, client-centred, solution-oriented co-operation project in vocational rehabilitation (SMVR intervention). METHOD: A same-subject study with a questionnaire was used, with 95 immediate superiors giving their views on co-operation both before and during the SMVR intervention. RESULTS AND CONCLUSIONS: The immediate superiors felt that the SMVR intervention was successful in promoting co-operation, and reported a significant decrease in referrals of employees from one organisation to another without the problem being resolved. Hence SMVR co-operation was experienced positively by the immediate superiors and in fact led to a joint responsibility in finding solutions. The immediate superiors saw greater possibilities than before for employees to resume regular or other jobs during the SMVR intervention. A more elaborate co-operation model such as the SMVR intervention increased the immediate superiors' experience of successful vocational rehabilitation. Organizations seeking to increase efficiency in vocational rehabilitation might well analyse their work methods and improve their forms of co-operation.

    In recent years researchers have tried to identify important factors that can explain re-integration to work for workers on long-term sick leave. The results indicate that multiple factors can explain whether or not people return to work. The aim of the study was to investigate long-term sick workers' experience of professional support for re-integration to work. Participants were drawn from the database of The National Social Insurance Board in Norway. A random sample was selected of persons with mental illness or musculoskeletal disorders (either p- or 1-diagnosis/symptoms related to International Classification in Primary Care), who were in receipt of Rehabilitation allowance. The questionnaire was sent to 1 493 persons of whom 740 responded. The most significant finding is that work training/mastering actions were identified by the majority of the rehabilitation benefit recipients as being of slight or moderate use. This highlights the importance of work training/mastering actions being relevant in proportion to the specific situation the individual is presently within. Delays in waiting for treatment attributed to approximately half of the rehabilitation benefit recipient's problems returning to work. This study also highlights that rehabilitation benefit recipients   experience loss of co-ordinated measures from those who are supposed to   contribute to simplifying their return to work.

    The main purpose of this study is to develop and test a generic model for workplace health management in organizations. Four private and four public organizations in northern Sweden were selected for the study. A model for health control was developed on the basis of a literature review and dialogues with the stakeholders in the workplaces. The model was then implemented at the workplaces during a two-year period. Interviews with leaders and sub-leaders were conducted on two occasions and were analyzed using content analysis and the constant comparison method. By using a grounded theory approach, three main categories were found: health closure and other health and working environment indicators, monetary accounting of health related indicators and changes in leadership behaviour and organizational practices. An important result was that the model influenced leadership values more than leadership and organizational methodologies. From the results a model for workplace health management is proposed, incorporating the planning, control, and improvement structures. The purpose of the model is to take health aspects into consideration when deciding organizational structure (work demands, control and social support). The model controls health by using health-related indicators with high frequency measuring whereas workplace health promotion is done in a structured way with a reflective model.

     Introduction: Statistical Process Control (SPC) charts have not been widely used to monitor workplace health and work environments. This research and effort to develop a more accurate and easy to use management control system for employee health is important from a humanistic, societal and economic standpoint, as well as complying with laws that regulate work environments. Objective: The purpose of the study is to design and discuss control charts as an early warning system for workplace health outcomes to promote workplace health management. Another purpose is to discuss relevant factors in the concept of the Out-of-Control Action Plan (OCAP) as a response when a chart warns that the workplace process may be malfunctioning. Participants: Two Swedish organizations were selected as case study organizations: a department at a university and an elderly care operation in a municipality. Methods: This study was explorative and should be seen as a starting point in learning how to use control charts for workplace health management. Self-assessed general health and new sick-cases per employee were selected as indicators for the control charts. Results: An integrated early warning system with Cumulative Sums-and Shewhart-charts are presented to show a possible method as to how an early warning system can be structured through the use of statistical control charts. Conclusions: The conclusion of this study is that control charts, along with well-designed implementation, make up a powerful and useable managerial early-warning system which promotes workplace health and helps to prevent sickness absence.

    Abstract [en]

    A problem attracting considerable attention in Sweden today is the substantial regional differences in sickness absence. The aim of the study was to investigate and compare how people, from a random sample of the population in both a rural area in the north of Sweden and the Swedish capital Stockholm, perceive their health, and what their attitudes are to work, leisure time and social welfare systems. Results showed that a larger proportion of those answering in Stockholm considered their health status to be "very good", compared with those in the rural area (p<0.0001). A majority in the rural area compared to the city of Stockholm reported a high or very high level of aches/pain (p<0.0001) and that work causes them physical problems p<0.0001). The population in both Stockholm and the rural area is of the opinion that the increase in sickness absence is mainly due to deterioration in the work environment. Almost half of the individuals in both the rural area and in Stockholm are of the opinion that many of those sick-listed are not actually ill. It may be that in the rural area in north Sweden people are more inclined to put their opinions to practice than those in Stockholm are

    Abstract [en]

    OBJECTIVE: Violent and disruptive acts committed by persons with developmental disabilities are a potential work environment problem for caregivers. This study developed and tested an instrument for describing and analyzing violence among persons with developmental disabilities. METHODS: Personnel from four units of one facility recorded daily on a simple form all violent and disruptive acts occurring in a limited range of situations. RESULTS: Acts of violence were associated with a small minority of individuals, but grossly underreported and unrecognized in formal reporting systems. None of the nearly 3000 incidents recorded during the six-week study required sick leave. Violent and disruptive acts occurred most often when an individual was requested to do something he or she did not want to do, when the individual was denied the opportunity to do something they did want to do and when waiting or under-stimulated. Personnel used the data in discussing countermeasures that could be integrated with the care and treatment of the individual. CONCLUSION: Hitting, kicking, pinching etc. of personnel are a work environment problem due to their high frequency and effects upon employee morale. The instrument and procedures can be used to reduce incidents of violence, which would improve the working environment of employees and assist persons with developmental disabilities as they move from institutional environments.

    Abstract [en]

    Working life continues to undergo rapid change. This change creates greater demand and sophistication and causes employees to experience more pressure, professionally and personally. Thus, absences from work due to sickness and injuries increase. In Sweden, this problem has become serious. This article argues that psychological empowerment and individual control are two key factors that minimize the many perils faced by the modern worker and those seeking to return to the work force through the vocational rehabilitation process. The findings show that a shift in ideology is needed. Specifically, there must be a shift from scientific management and Weberian bureaucracy towards organizational structures, routines, and cultures that support and increase individual worker psychological empowerment and control.

    Objective: To investigate and gather knowledge of how important the various functions of Swedish rehabilitation workers are and whether this knowledge is equivalent to the demands of the vocational rehabilitation process. Participants: Swedish rehabilitation workers from both public and private sectors.

    Methods: A questionnaire comprising 89 work task items was used to identify the major dimensions of the Swedish rehabilitation workers job in the rehabilitation process. A principle component factor analysis was performed on the 89 job-task items from the Swedish Task Inventory.

    Results: The number of factors/dimensions that could be formed was only three, which is judged to be limited both in scope and depth. The level of perceived knowledge reported varied from limited to having the required competence. The results also reveal that the workers perceive their work tasks and the dimensions that make up their role function as being of moderate to low importance. Their perceived level of knowledge for the similar work task items and dimensions were rated moderate to low. Differences between the groups of rehabilitation workers were mixed.

    Conclusions: The vocational rehabilitation process offered in Sweden is limited. It appears that the level of knowledge within the area can and should be increased and that better education is needed in order to improve the quality of the vocational rehabilitation process and for further development to take place.

    BACKGROUND: Absence from work due to musculoskeletal disorders is a significant problem from a number of perspectives, and there is a great need to identify factors that facilitate return to work (RTW). OBJECTIVE: To identify factors related to RTW after vocational rehabilitation for sick-listed workers with long-term back, neck and/or shoulder problems, and to compare the results with those from a previous literature review based on studies from 1980 to 2000. METHODS: A literature review based on studies published 2001-2014 in PubMed, CINAHL, EMBASE, ERIC and the Cochrane Library. RESULTS: Major risk factors for not returning to work are: higher age, factors related to pain such as higher levels of pain and pain related fear, avoidance of activity, high distress and depression. Facilitating factors for RTW are: lower functional disability, gaining control over one's own condition, believing in RTW and work-related factors such as occupational training, and having a job coach or an RTW coordinator. Compared with the results from the previous review, the present review shows less of a focus on sociodemographic factors and more on psychological factors. Both studies highlight risk factors such as older age, higher levels of pain, depression and less internal locus of control. CONCLUSION: Psychosocial and work-related factors are important and should be included in interventions for the RTW of people with long-term back, neck and/or shoulder problems.

    BACKGROUND: In aging societies, zest for work may be pivotal when deciding to stay occupationally active longer. Psychosocial work stress is a prevalent public health problem and may have an impact on zest for work. Work over-commitment (WOC) is a personal coping strategy for work stress with excessive striving and a health risk. However, the long-term effect of WOC on zest for work is poorly understood. OBJECTIVE: To investigate the age-related associations of work over-commitment with zest for work. METHODS: During 1996-1998 and 2000-2003, predominantly industrial workers (n = 2940) participated in the WOLF-Norrland study and responded to a questionnaire referring to socio-demographics, WOC, zest for work, effort-reward imbalance proxies, and mental health. Age-adjusted multiple logistic regressions were performed with original and imputed datasets. RESULTS: Cross-sectionally, work overcommitted middle-aged employees had an increased prevalence of poor zest for work compared to their contemporaries without WOC (OR: 3.74 [95%-CI 2.19; 6.40]). However, in a longitudinal analysis associations between onset of 'poor zest for work' and the WOC subscales 'need for approval' (OR: 3.29 [95%-CI 1.04; 10.37]) and 'inability to withdraw from work' (OR: 5.14 [95%-CI 1.32; 20.03]) were observed. CONCLUSION: The longitudinal findings among older employees could be relevant regarding the expected need to remain occupationally active longer.

    PURPOSE: The aim of the current study was to investigate whether Case Management and the strengths model could be a successful method in vocational rehabilitation of unemployed people on long term sick leave in Sweden. An additional aim was to further highlight the actual rehabilitation process. MATERIAL AND METHOD: Ten clients, five men and five women aged between 35 and 40, were included in the project. All ten were at the project start both long-term unemployed (2-13 years) and on long term sick leave (2-6 years). The project was evaluated from four perspectives: Quality of life (SF 36), sickness absence, and contact with working life were investigated before and after the project. The rehabilitation process was investigated during and after the project. RESULTS AND CONCLUSION: The result from the study was positive. Of the 10 clients who participated in the project, six have improved their quality of life, seven have a diminished sickness absence and seven have a closer contact to working life after the project than before. With regards to the rehabilitation process, the results here also impart a positive picture. It should be noted, however, that the study is small and not based on a random sample. The results should not be generalized, but recognized instead as a positive indication.

    In previous studies, internal locus of control (ILC) has been pointed out as a

    key factor for return to work after vocational rehabilitation. The aim of the

    current study was to gain a deeper understanding of the concept of ILC in a

    Swedish vocational rehabilitation context. The study was based on data from

    347 long-term sick-listed clients collected at the onset of vocational

    rehabilitation. A first bi-variate analysis showed that ILC was positively

    associated with physical functioning and general health, and negatively

    associated with bodily pain. The analysis also showed that women, more than

    men, reported high internal locus of control. After a second multivariate

    analysis, only bodily pain remained associated. It is concluded that there

    exist a strong and negative association between bodily pain and internal

    locus of control. Clients with severe pain often also suffer from low internal

    locus of control. This should be kept in mind when providing vocational

    rehabilitation.

    Sick leave has negative economic consequences for companies. The aim of this study was to implement the model Organizational Rehabilitation in two departments at an industrial company in the county of Jämtland, Sweden, to see if it was possible to reduce the costs due to sick leave. The rehabilitation model consisted of three steps and combined economy and productivity with a good psychosocial work environment, measuring the result in costs. Step 1 consisted of a problem inventory among all co-workers, step 2 semi structured interviews with all co-workers regarding how they experience their work environment and step 3 individual agreements regarding rehabilitation interventions, i.e. interventions where the individual's specific needs and wishes outlined the foundation for rehabilitation measures with the objective of increasing work capacity. The costs were measured at the start and end of the project. The study showed that it is indeed possible to reduce both sick leave and costs. During the study, the organization reduced current sick leave from 12% (2003) to 3% (Nov. 2004) and the costs per employee from euro1 860 per month (2003) to euro440 per month (Nov. 2004). This will make an estimated saving for 2005 of euro470 000. It should be noted that the study is small and should not be generalized, but may be seen as a positive indication regarding the model organizational rehabilitation. Further research is needed.

    OBJECTIVE: The aim of this study was to perform a 5-year follow-up of a 7-week multidisciplinary rehabilitation program focusing on pain, physical disability, anxiety and depression as well as stress and sick leave and to compare the results from the baseline, a 2-year follow-up and this 5-year follow-up. PARTICIPANTS: Out of 60 participants of the original rehabilitation program 54 were followed-up after 5 years. METHODS: The rehabilitation program was individually adapted and consisted of physical activity in several forms as well as theoretical and practical education. At baseline (start of study) and 2 and 5 years after completion of the rehabilitation program all participants were evaluated in terms of sick leave, pain rating (Visual Analogue Scale), the Disability Rating Index, Hospital Anxiety and Depression Scale, and Stress Test. RESULTS: Between the start of the program and the 5-year follow-up pain rating (P < 0.017) and the rate of full-time sick leave (P < 0.0005) decreased. Physical disability, anxiety, depression and stress were maintained from the 2-year follow-up until the 5-year follow-up. CONCLUSIONS: The rehabilitation program seemed to have had an effect on the participant's ability to manage with symptoms long after the end of the rehabilitation program. Most participants had returned to work, and reported less pain. The improvements made in physical disability and mental health prior to the 2-year follow-up were maintained at the 5-year follow-up occasion.

    PURPOSE: To investigate whether improvement of range of motion (ROM) in persons with musculoskeletal disorders, mainly neck and back pain, was associated with a favourable development regarding physical disability, pain, and health-related quality of life (QoL), and whether such development differed between sick-listed and non-sick-listed persons during and up to 2 years after a multidisciplinary rehabilitation programme.

    METHODS: Ten persons with full-time sick leave (Group I) and 49 with part-time or no sick leave (Group II) at the end of a previous study participated. It was shown in that study that Group I had higher pain rating and higher subjective physical disability than Group II, with little or no improvement during and after rehabilitation. In the present study, all participants were evaluated with neck and back mobility tests: Disability Rating Index (DRI); Pain Intensity Rating on a visual analogue scale (VAS); and Global Self-Efficacy Index (GSI).

    RESULTS: Cervical and thoracolumbar spine ROM were lower in Group I than in Group II from the start of rehabilitation to a 2-year follow-up. Only Group II showed a temporal improvement in ROM. No changes in DRI, VAS or GSI were found in parallel with corresponding temporal changes in any of the ROM.

    CONCLUSION: Group II but not Group I improved in active ROM during rehabilitation; further, in Group I active ROM in the cervical and thoracolumbar spine did not improve during the 2-year follow-up. Improvement of ROM showed no correlation with physical disability, pain or QoL.

    Objective: The aim of this qualitative study was to explore experiences of sick-listed persons of not returning to work during a six-year period after participation in an extensive multidisciplinary rehabilitation programme. Participants: The study comprised ten participants with musculoskeletal disorders, mainly neck and back pain. Methods: Semi-structured interviews were conducted and analysed by manifest content analysis. Results: This led to identification of three primary categories and six sub-categories, which described the participants' experiences of barriers to and possibilities of returning to work, and indicated what strategies they used to cope with everyday life. The participants described that the main barriers to returning to work were pain and somatic symptoms, fatigue, and not fulfilling the work requirements. Participants considered physical activity a key factor in coping with pain. Most participants thought that they had residual work ability, and could utilise this if they could get a modified job adapted to their own capacity. Conclusions: Our study highlights the importance of utilising residual working ability at workplaces. Finding flexible work possibilities requires an understanding and supportive attitude on the part of both the employer and the social insurance office.

    The concept of work ability is not clearly defined although it has a central place in vocational rehabilitation. Several health professions are involved in assessing work ability, physicians and occupational therapists are two of these. OBJECTIVE: The purpose of this study was to explore occupational therapist and physician views about work ability and experiences in assessing work ability. PARTICIPANTS: Fourteen physicians and 23 occupational therapists participated in seven focus group discussions that were audio taped and transcribed verbatim. METHODS: Qualitative content analysis was used. Each author performed an individual preliminary analysis. These analyses were later discussed and refined in the research team and a workshop. The final categorization resulted in one theme, four categories and 13 sub-categories. RESULTS: The overall theme expressed work ability as an obscure, complex and unique concept. The four categories illustrate the affecting factors and confirm the complex structure of work ability: the person, the context of life, the work, and the society. Physicians expressed greater difficulty in assessing work ability than occupational therapists did, because they have fewer instruments to access this concept. CONCLUSIONS: Assessment of work ability requires team cooperation with several different professionals. Cooperation could increase accuracy in issuing sickness certification and strengthens the ability of identifying individual requirements for rehabilitation.

    The problem of violence at work has received considerable attention in recent years. According to Swedish statistics, personnel in the health-care sector are the most affected. The purpose of this paper is to present a framework or model for studies of workplace violence, and to demonstrate its application to the analysis of violence in health-care settings. The model treats violence as a process comprising of several stages/sub-events, and shows how this process is influenced by underlying structural (rather permanent) and situational (generally temporary) risk factors. Understanding the diversity in the process provides a basis for analysis of connections between interacting factors at different stages. Risk management (hazard identification, risk analysis, and preventive counter-measures) should be integrated into the work situation and operations of an organization.

    Abstract [en]

    This article has a two-fold approach. First, it investigates relationships between work organizational factors, and health and performance outcomes. Second, it compares two change strategy approaches in workplace health interventions by studying changes of these factors and outcomes. The sample consisted of ten Swedish small enterprises including 102 individuals, who answered a before and after questionnaire about organizational factors and outcomes. The leaders were interviewed and answered a questionnaire about performed workplace health interventions. Statistical methods used were reliability tests, correlation analyses and t-tests. Results indicate rather strong links between indicators of respectful leadership, creative work and team spirit, and the outcome indicators self-assessed health and judged workplace adaptability in association with customer satisfaction. The results concerning changes of determinants and outcomes (after workplace health interventions) showed significant differences between enterprises using a broad change strategy and those using an expert/problem-based strategy with the former having more favourable results. The leader interview results also point at obstacles concerning workplace change processes as lack of resources, insufficient competence and influence of external factors. The study results suggest that work organizational factors and integrated models for workplace health intervention are of importance for health and performance outcomes in small enterprises.

    This article focuses on relationships between organizational factors such as leadership, learning, psychosocial work environment and quality aspects as they relate to organizational and health performance outcomes in 42 small enterprises in Norway and Sweden. A rather explorative analysis model was created using indicators that were based on theoretical concepts from a literature review and questionnaire data, concerning 988 employees and leaders. These indicators were then used for correlation analysis. The enterprise is the unit of analysis. Some strong links between organizational factors and organizational and health performance were found. There was also a strong relation between health performance and sickness absence. Using structural analyses, a structure containing six general components, and strong interrelationships between some indicators of organizational performance and health performance were found. It was also possible to position the enterprises according to two general dimensions. As a general guideline for action, the results tend to support the perspective that positive organizational development is related to health performance and a lowering of absence due to sickness. The findings support the strategies of practitioners who use a concept-driven or holistic approach that integrates several facets of workplace development.

    BACKGROUND: Leaders and co-workers in Swedish public sector organizations are exposed to demanding psychosocial working conditions; more knowledge about workplace-based interventions in this sector of working life is needed. OBJECTIVE: To compare co-workers’ and leaders’ self-ratings of health and psychosocial working conditions, and investigate how prevention and rehabilitation in Swedish public sector workplaces affects these ratings. METHOD: The longitudinal panel data consisted of 311 individuals (20 leaders, 291 co-workers) at 19 workplaces. Based on questionnaire data, statistical analyses were performed using Mann-Whitney U-Test, pair-wise Spearman correlations, a mixed between-within subjects ANOVA and Friedman’s test. RESULTS: Results indicate differences in how the leaders and the co-workers judge their health and psychosocial working conditions. Leaders report work content that is more varied and interesting as well as more possibilities for personal development through work, yet they also report more tiredness, concern over managing their work situation and time pressure at work. Comparisons of mean values for used indicators show some improvements after one year, but also several non-significant or negative time trends two years after the interventions were initiated. CONCLUSION: The study provides some support for experienced differences between co-workers’ and leaders’ health and psychosocial working conditions in public sector workplaces, indicating the importance of different workplace-oriented prevention and rehabilitation interventions for these two categories of employees.

    Objective: The purpose of this study is to identify situations and phenomena that have simplified returning to work for women on long-term sick leave. Participants: Seven women who were exposed to a relatively large number of risk factors that normally are associated with difficulties in returning to work. Methods: In-depth interviews with qualitative content analysis. Results: The analysis indicated four main categories of factors: The Individual, Interactions, Surrounding Resources, and Situations. In each of the main categories structural factors exist and it appears that these have been of significant importance to the women in their return to work. These are presented as Key Factors and they are: clarification of - and the need for - support in the personal process of change; desire, longing, and vanity; respectful interactions between the individual and people in her surroundings; the structure and content of the rehabilitation clinic; the importance of the perceived reality; and the individual's sense of control during the work related rehabilitation process. Conclusions: The results mostly revealed phenomena that have been indicated and described in earlier research studies. However, emotions such as desire, longing and vanity as motivation and driving forces behind a return to work have not been earlier described.
