    Abstract [en]

    We evaluated the interrater reliability and concurrent validity of the HCR-20 Version 3 (HCR-20V3). Three sets of ratings were completed by experienced clinicians for 35 forensic psychiatric patients, for both HCR-20 Versions 2 and 3. Reliability analyses focused on ratings of the presence of Version 3 risk factors, presence of Version 3 risk factor sub-items, relevance ratings for Version 3 risk factors, and Version 3 summary risk ratings for future violence. Concurrent validity analyses focused on the correlational association between Versions 2 and 3 in terms of the number of risk factors present. Findings indicated that Versions 2 and 3 were strongly correlated (.69 -.90). Interrater reliability was consistently excellent for the presence of risk factors and for summary risk ratings. The majority of relevance and sub-item ratings were in the good to excellent range, although there was a minority of such ratings in the fair or poor categories. Findings support the concurrent validity and interrater reliability of HCR-20V3. Implications for use of HCR-20V3 by professionals and agencies are discussed. © 2014 International Association of Forensic Mental Health Services.

    Abstract [en]

    The HCR-20 Version 3 (HCR-20V3) was published in 2013, after several years of development and revision work. It replaces Version 2, published in 1997, on which there have been more than 200 disseminations based on more than 33,000 cases across 25 countries. This article explains (1) why a revision was necessary, (2) the steps we took in the revision process, (3) key changes between Version 2 and Version 3, and (4) an overview of HCR-20V3's risk factors and administration steps. Recommendations for evaluating Version 3 are provided. © 2014 International Association of Forensic Mental Health Services.

    Introduktion: Medling med anledning av brott har funnits i Sverige sedan 1980- talet då ungdomsbrottsligheten betraktades som ett ökande samhällsproblem. År 1997 fick Brottsförebyggande rådet (Brå) i uppdrag av regeringen att stötta och främja olika medlingsprojekt runt om i Sverige och idag är medling lagstadgat, bland annat ska alla kommuner i Sverige sörja för att kunna erbjuda medling till samtliga lagöverträdare under 21 år. Metod: Studien var av kvalitativ ansats och utfördes genom åtta semistrukturerade telefonintervjuer med medlingssamordnare i Sverige. Respondenterna representerade fyra olika kommunstorlekar och intervjumaterialet analyserades genom en deduktiv innehållsanalys. Syfte: Syftet med studien var att belysa förutsättningar och begränsningar med att bedriva medlingsverksamhet i svenska kommuner. Resultat: Den främsta förutsättningen för att kunna bedriva medlingsverksamhet var medlingssamordnares uppfattningar om medlingens positiva påverkan på brottsdrabbade och gärningspersoner. Bristfälligt ärendeflöde, avsaknaden av nationella riktlinjer om hur arbetet borde organiseras samt bristande samverkansrutiner med andra aktörer uppgavs som betydande begränsningar. Diskussion: Medlingsverksamheter i Sverige organiserades på olika sätt i olika kommuner vilket kunde bero på den ramlagstiftning som finns om medling. Det varierande ärendeflödet kunde ses som ett resultat av bristande kunskap om medlingens syfte och effekter. För att medlingsverksamheterna ska bedrivas rättssäkert föreslog författarna till den aktuella studien ett tydligt centralt ansvar för verksamheterna samt att riktlinjer och direktiv tydliggörs. Författarna ansåg också att en fungerande medlingsverksamhet i Sverige var viktig då tidigare forskning rapporterat att medling som intervention har minskat återfall och ökat välmående hos såväl gärningspersoner som brottsdrabbade.

    Bakgrund: När en familjemedlem blir svårt sjuk har det en stor påverkan på hela familjen. Närstående har och känner ofta ångest, depression och maktlöshet. Familjemedlemmens sjukdom kan också leda till att rollerna i familjen ändras, och en förändring i den ekonomiska situvationen och en ökad påfrestning för familjen. Syfte: Syftet med studien var att belysa behov som närstående till patienter inom palliativ vård kan ha. Metod: En litteraturstudie genomfördes med sökningar i Pubmed och Cinahl. Innehållsanalys gjordes av de 11 inkluderade artiklarnas huvudresultat. Tydliga framträdande fenomen i texten kondenserades, kodades och katogoriserades i underkategorier och kategorier. Resultat: I studiens resultat framkom att närstående till patienter inom palliativ vård har ett stort behov av kommunikation, information och stöd. Diskussion: Att ha en fungerande kommunikation fanns var något som var viktigt för de närstående, då det gav dem en känsla av trygghet. Det var även viktigt att rätt infomation gavs till de närstående, att den gavs vid rätt tillfällen och att de närstående kunde förstå informationen.För att stödja närstående spelar vårdpersonal en viktig roll, då de kan bidra med uppmuntran och hopp i en svår situation.

    Research on how to recover from athlete burnout is scarce. The current aim is therefore to describe an intervention with an elite shooter suffering from burnout, and the use of mindfulness and Qigong to reestablish sport functioning as well as general well-being. The participant used mindfulness and Qigong exercise on a daily basis. Exercise frequency, exercise time, concentration level and Qigong state were noted daily, and levels of stress, energy and primordial force were self-rated weekly for 20 weeks, and followed up after 30, 40 and 50 weeks. The participant recovered from burnout to a state of general well-being (energy and primordial force changed from weak to strong), and her ability to stay concentrated in a Qigong state changed from weak to strong. Her capacity to shoot high scores was reestablished, even if her shooting endurance was not fully recovered. Mindfulness and Qigong techniques may be useful in the prevention of and recovery from athlete burnout.

    Bakgrund: Vårdpersonalen har en viktig funktion i omvårdnaden av närstående till palliativt sjuka patienter. Den palliativa vårdsituationen var ofta en ny upplevelse för de närstående. Vårdpersonalens uppgift var därmed att ge aktuell information samt se till att närstående blev delaktiga i vården. Syftet: Med litteraturstudien var att belysa vårdpersonalens stöd till närstående samt beskriva vilka behov närstående hade i palliativ vård. Metod: Femton vetenskapliga artiklar med kvalitativ metod analyserades enligt innehållsanalys och låg till grund för litteraturstudien. Resultat: Det framkom att vårdpersonal och närstående ansåg att de viktigaste funktionerna var att ge och få stöd. Närstående önskade information och kunskap om den svårt sjukes tillstånd samt sjukdomsförlopp. Slutligen visade resultatet att det var viktigt när personalen tog sig tid att lyssna och tala med de närstående samt att alla parter var delaktiga i de beslut som togs. Diskussion: Genom stöd från vårdpersonalen blev den svåra situationen för de närstående hanterbar. Den viktiga informationen ledde till begriplighet över den nya situationen, där delaktigheten i vården av den svårt sjuke skapade en meningsfullhet för de närstående. Konklusion: Vårdpersonal borde få mer kunskap och träning i att ge information och omvårdnad till närstående för att kunna tillgodose deras behov.

    Bakgrund: Våld i nära relation är ett allvarligt och utbrett samhällsproblem. Våldsutsatta kvinnor kan söka hjälp överallt inom vården. Sjuksköterskors erfarenheter av att möta våldsutsatta kvinnor kan vara avgörande för identifiering och adekvat fortsatt vård. Syfte: Litteraturöversiktens syfte var att beskriva sjuksköterskors erfarenheter av att möta kvinnor som utsatts för våld i nära relation. Metod: Litteraturöversikt baserad på vetenskapliga artiklar sökta i databaserna PubMed och Cinhal.Totalt tolv kvalitativa och kvantitativa artiklar har kvalitetsgranskats, analyserats och sammanställts till ett resultat. Resultat: Sjuksköterskor ansåg att samtliga kvinnr bör screenas för våld i nära relation. Faktorer som hindrade sjuksköterskor från att utföra screening var; tidsbrist, brist på avskilt utrymme för samtal, otillräcklig utbildning och kunskap. Inställningar och attityd kunde vara avgörande för vilken hjälp och stöd som de våldsutsatta kvinnorna fick. Diskussion: Samtal i avskild miljö med kvinnor som misstänkts ha varit utsatta för våld i nära relation borde vara möjligt för sjuksköterskor. Det fanns ett ökat behov av kunskap för att kunna ge adekvat vård till kvinnor som utsatts för våld i nära relation, vilket borde tillgodoses av arbetsgivaren. Slutsats: Att medvetandegöra sjuksköterskor om våldsutsatta kvinnor kan bidra till kunskap , förbättra hjälp och stöd till dessa kvinnor. Förbättringar behövs inom området, till exempel riktlinjer och vårdprogram, som kan utgöra ett värdefullt stöd för sjuksköterskor.

    "Struggling for independence'': The meaning of being an oldest old man in a rural area.     Abstract [en]

    The amount of older people receiving home nursing care is increasing; in rural areas, they are at additional risk because of the distance between people and health care facilities. No specific studies have been found about oldest old men living alone and receiving home nursing care and the meaning of living alone in one's own home. The aim of this study was therefore to illuminate the meaning of being an oldest old man living alone in a rural area and receiving home nursing care. A sample of 12 oldest old men living in rural areas in the middle of Norway was chosen for this study. Narrative interviews were conducted, and data were analyzed using the phenomenological hermeneutical method. After a naive reading and a structural analysis of the text, we identified three themes: feelings of insufficiency in everyday life, finding hope in life, and feeling reconciliation with life. The comprehensive understanding suggested that being an oldest old man living alone in a rural area means a struggle between a dependent existence and a desire to be independent. Living in the tension between independence and dependency is a complex emotional situation where one is trying to accept the consequences of life and loss-reconciling the wish to live with the fact that life will come to an end.

    BAKGRUND: En konfrontation och möte med döden sker för de flesta genom ens egen eller en närståendes svåra sjukdom. Att vara anhörig till en svårt sjuk person är inte en enkel roll och för många anhöriga kan detta vara en ny och skrämmande situation. Samtidigt som den anhörige förväntas finnas där och stödja den sjuke, bär denne också på en egen sorg. SYFTE: Att belysa närståendes upplevelser vid vård i livets slutskede av en anhörig i hemmet. METOD: Litteraturstudie med 15 artiklar inkluderade, materialet analyserades med innehållsanalys. RESULTAT: Analysen mynnade ut i fem kategorier samt 21 subkategorier. Resultatet visade på att anhöriga upplevde en rad negativa känslor i samband med vårdandet av en svårt sjuk eller döende anhörig, men också positiva känslor som till exempel tillfredställelsen av att veta att ha gjort allt för den sjuke. Stöd var en viktig komponent i vårdandet av en anhörig, stödet kunde både komma från vårdpersonal, familj och vänner. DISKUSSION/SLUTSATS: Att se även den anhörige i vårdandet av en svårt sjuk i hemmet, borde vara av stor vikt för sjukvårdspersonal för att kunna lindra både den sjukes och dess anhörige lidande.

    Background When cure is no longer possible, medical care should aim for a transition to palliative care regardless of disease. Patients with incurable cancer are often treated with palliative chemotherapy (PCT), starting with the intent to prolong life and increase quality of life. Eventually, in the late stages of the disease, the patient reaches a transition phase when further PCT neither prolongs life nor adds any predominantly positive effects.

    Aim of the thesis

    Study I: To analyse the proportion of patients with incurable cancer who received palliative chemotherapy during the last month of life, and to identify their discriminative characteristics.

    Study II: To develop a questionnaire assessing performance status in palliative chemotherapy, and to test its psychometric properties.

    Study III: To explore challenging situations experienced by registered nurses when administering palliative chemotherapy to patients with incurable cancer.

    Study IV:  To investigate whether routine use of the Performance Status in Palliative Chemotherapy (PSPC) questionnaire in PCT would affect the proportion of patients receiving PCT during the last month of life, hospital admissions, notifications of performance status, documented decisions of ceasing PCT in the medical records, and/or place of death. A secondary aim was to gather registered nurses’ experiences of PSPC in clinical use.

    Methods In Studies I and IV, information from the medical records of deceased patients with epithelial cancers was used in descriptive analyses of the proportions of patients receiving PCT in counties in northernmost Sweden. A quantitative design was chosen, using non-parametric statistical methods. In Study II, a brief patient-completed questionnaire assessing performance status was developed and psychometrically tested. In Study III, data from research interviews with registered nurses were analysed qualitatively with a narrative thematic approach.

    Results Studies I and IV showed that about 25% of patients receiving PCT were treated during the last month of life. This group of patients had more hospital admissions, were less likely to die at home, and had fewer instances of documentation of the decision to cease PCT. The questionnaire developed in Study II was shown to have acceptable psychometric qualities such as reliability, validity, and sensitivity to detect deterioration in performance status. Study IV showed that the questionnaire gave nurses valuable information about patients’ performance status. The results also showed that 97% of nurses and 48% of physicians documented their patients’ performance status in the medical records. Study III demonstrated that when nurses administered PCT they considered futile, they could experience dilemmas created by the unforeseeable outcomes of PCT or stemming from insufficient communication between nurses, patients, next-of-kin, and physicians.

    Conclusions Administration of PCT can create dilemmatic situations for both the patient and medical staff when approaching end-of-life. This is underlined by the finding that some 25% of treated patients received their last round of PCT as late as during the last month of life. The decisions to cease PCT were less likely to be documented for patients who had received PCT within a month before death. Nurses described situations where they felt they were in the middle of the decision-making process regarding whether or not to continue PCT. They found the treatments were given on the authority of someone else; the physician’s recommendation or the patient’s and/or relatives’ request.

    The unpredictability of PCT was a continuous theme in the work described in this thesis, emphasizing the necessity of individually assessing every patient before PCT in order to minimize the risk of futile treatments. The attempt to develop a reliable and valid questionnaire for systematic assessment of performance status has increased future possibilities to monitor this parameter in PCT when approaching end-of-life. The questionnaire developed as part of this thesis has provided nurses with increased knowledge of patients’ performance status. If routinely used, it may help decrease the proportion of patients receiving PCT during the last month of life, though this remains to be rigorously proven. Further research efforts are needed to progress in the task of optimizing rather than maximizing the use of PCT when approaching end-of-life.

    BACKGROUND: Earlier studies have shown that up to 43% of patients with incurable cancer are treated with palliative chemotherapy in the last month of their lives. Although pretreatment blood tests are acceptable, the patient's general condition may not permit further palliative chemotherapy treatment (PCT). Presently, there is no patient self-assessment tool available to monitor performance status during PCT.

    OBJECTIVES: To describe the development process of the Performance Status in Palliative Chemotherapy (PSPC) questionnaire, and the testing of its psychometric properties.

    METHODS: The questionnaire was developed by the authors based on the Eastern Cooperative Oncology Group Performance Status Rating (ECOG PSR) scale as well as their clinical experience with PCT. Adult patients who were diagnosed with epithelial cancers (n = 118) were enrolled to test the PSPC questionnaire for reliability, sensitivity for change, and validity.

    RESULTS: After stepwise modifications of the PSPC questionnaire, psychometric tests revealed acceptable values for reliability (via a test-retest method), sensitivity for change (via a comparison of patients with progressive disease over time), and validity (via a comparison of the PSPC vs the Edmonton Symptom Assessment System [ESAS]).

    LIMITATIONS: At this stage of questionnaire development, we are unable to conclude whether the PSPC is superior to the conventional ECOG PSR in the evaluation of performance status and the prediction of chemotherapy response.

    CONCLUSION: Psychometric tests suggest that the PSPC questionnaire may be a useful patient-completed tool in the late stages of cancer disease to routinely monitor performance status in palliative chemotherapy treatments so as to minimize the risk of inflicting more harm than good.

    Palliative chemotherapy treatments (PCT) are becoming more common for patients with incurable cancer; a basic challenge is to optimize tumour response while minimizing side-effects and harm. As registered nurses most often administer PCT, they are most likely to be confronted with difficult situations during PCT administration. This study explores challenging situations experienced by nurses when administering PCT to patients with incurable cancer.

    Methods

    Registered nurses experienced in administering PCT were asked in interviews to recall PCT situations they found challenging. Inspired by the narrative tradition, stories were elicited and analysed using a structural and thematic narrative analysis.

    Results

    A total of twenty-eight stories were narrated by seventeen nurses. Twenty of these were dilemmas that could be sorted into three storylines containing one to three dilemmatic situations each. The six dilemmatic situations broadly related to three interwoven areas: the uncertainty of the outcome when giving potent drugs to vulnerable patients; the difficulty of resisting giving PCT to patients who want it; and insufficient communication between nurses and physician.

    Conclusion

    Nurses who administer PCT are engaged in a complex task that can give rise to a number of dilemmatic situations. The findings may be interpreted as meaning that at least some situations might be preventable if the knowledge and insight of all team members – nurses, physicians, patients, and relatives – are jointly communicated and taken into account when deciding whether or not to give PCT. Forming palliative care teams early in the PCT trajectory, could be beneficial for staff and patients.

    Background: In situations of care transfer of older people from hospital to home care at discharge, exchanging relevant and necessary information about the patient’s health status and individual needs are of importance to ensure continuity and appropriate nursing follow-up care. Objective: The objectives of the study were to: 1) examine the content of nurses’ discharge notes of older patients’ discharged from hospital to home care, and 2) investigate the association between the content of discharge notes and characteristics of patient and transfer. Methods: The nursing discharge notes of 70 older patients admitted to a geriatric unit and a general medicine ward at a local hospital in central Norway were analysed. The discharge notes were structured in accordance with the Well-being, Integrity, Prevention, and Safety (VIPS) model. Mean, standard deviations, and independent sample t-tests were performed to show and examine differences in use of VIPS keywords in relation to patient and transfer characteristics. To examine if use of VIPS keywords could be predicted by patient and transfer characteristics, linear multiple regression analyses were used. Results: Significant differences for mean scores on used VIPS keywords in the discharge note were found for gender, age, and medical department facility. While gender and medical department facility were significant predictors of mental related keywords in the discharge note, medical department facility was a significant predictor of physical related keywords. Conclusions: The result of this study indicate that documentation of patient status in the nursing discharge note of older patients transferred from hospital to home care is incomplete and are influenced by patient and transfer characteristics. In order to ensure continuity and appropriate nursing follow-up care, we emphasize the need for a more comprehensive approach to older patients, and that this must be reflected in the nursing discharge note.

    Detta avhandlingsarbete är utfört inom en rättspsykiatrisk vårdkontext och återspeglar patienters[1] och personals[2] erfarenheter och upplevelser av vändpunkter i samband med minskad risk för våld och återhämtning. Med utgångspunkt från ett holistiskt och salutogent hälsovetenskapligt perspektiv har avhandlingen ett tvärvetenskapligt förhållningssätt.   Teoretiska ämnesdiscipliner såsom psykiatrisk omvårdnad, kriminologi och sociologi har influerat arbetet.

    Avhandlingens övergripande syfte var att utifrån de salutogena aspekterna i det rättspsykiatriska omvårdnadsarbetet undersöka vad som är relaterat till vändpunkter i samband med minskad risk för våld och vad som bidrar till återhämtning. Avhandlingen baseras på fyra olika studier. Syftet med den inledande kvantitativa delstudien (I) var att identifiera och jämföra rättspsykiatriska patienter som sänkt sin bedömda risk för våld med 30 % eller mer enligt riskbedömningsinstrument HCR-20. Resultatet visade att den bedömda risken för våld minskade över tid. En demografisk analys genomfördes för att studera skillnader mellan de patienter som sänkt sin bedömda risk för våld och de som inte sänkt sin risk. Det framgick att rättspsykiatriska patienter som bedömts med hjälp av riskbedömningsinstrumentet HCR-20, minskade den bedömda risken för våld, både på kort och på lång sikt. Den rättspsykiatriska vården fungerade bäst när det gällde att förbättra de kliniska riskfaktorerna (C-skalan).

    Riskhanteringen (R-skalan) gällande eventuell utskrivning och framtida friförmåner visade inte samma goda progress. Demografiska karaktäristika såsom ålder, alkohol och drogmissbruk och psykiatriska diagnoser var inte relaterade till minskad risk för våld, dock var kvinnliga patienter och patienter utan psykopatidiagnos mer benägna att sänka sin risk för våld. Ett urval av de patienter som minskade sin risk för våld med 30 % eller mer utgjorde basen för delstudie II och III.

    I delstudie II intervjuades tretton rättspsykiatriska patienter om upplevelser och erfarenheter kring vad som bidrar till minskad respektive ökad risk för våld inne på en rättspsykiatrisk avdelning. Data analyserades med hjälp av en tolkande beskrivning (Interpretive Description). Studien visade att orsakerna till ökad respektive minskad risk för våld var processrelaterad, där interaktioner mellan personal och patienter bidrog till en utveckling som antingen präglades av välbefinnande eller disharmoni. Delstudie II utgjorde en viktig kunskapskälla som präglade ansatsen i delstudie III och IV. Detta med hänvisning till att flertalet patienter i delstudie II lämnade intressanta beskrivningar av vändpunkter i samband med minskad risk för våld och återhämtning.

    Sålunda intervjuades i delstudie III, tio rättspsykiatriska patienter om deras upplevelser av förändringsprocesser i samband med vändpunkter och återhämtning. Dataanalysen skedde med hjälp av en kvalitativ innehållsanalys. Processen beskrevs utifrån tre faser där man i högriskfasen upplevde kaotiska och överväldigande känslor. Vändpunkten upplevdes som ett känsligt skede som präglades av att tvingas hitta en ny, konstruktiv väg i livet. Återhämtningsfasen präglades av ett accepterande och en mognad. Stöd och erkännande från omgivningen ansåg främja dessa processer.

    Delstudie IV bestod av intervjuer med tretton personal. Syftet var att belysa upplevelser och erfarenheter kring rättspsykiatriska patienters vändpunkter och återhämtning. Kvalitativ innehållsanalys användes för att analysera data. Beskrivningar kring hur en vändpunkt kunde bevaras och främjas baserades på vikten av att vara uppmärksam, lyhörd och att inte skynda på processen. En vändpunkt upplevdes genom att patienten uppvisade synbara positiva förändringar. Sammansättningen på personalen och patienterna ansågs påverka atmosfären på avdelningen, och bidrog till huruvida processen mot vändpunkter och återhämtning underlättades eller försvårades.

    Avhandlingen visar hur ett ständigt växelspel mellan patienter och mellan personal bidrog till huruvida vårdatmosfären upplevdes såsom hälsofrämjande eller ej. Vidare framgår att processer kring vändpunkter i samband med minskad risk för våld och återhämtning präglades av känslor av sårbarhet och utsatthet. Processen beskrevs som ett känsligt förlopp som behövde understödjas av förtroendefulla relationer med andra, samt möjligheter att få vistas i en trygg miljö.

    [1] Den vetenskapliga litteraturen använder sig av begrepp såsom client, consumer, service user eller forensic patient för att beteckna människor som är inskrivna inom den rättspsykiatriska vården. Inom det psykiatriska svenska fältet kan begreppen brukare, vårdtagare eller patient förekomma. I denna avhandling kommer begreppet patient och vårdtagare omväxlande att användas för att beteckna de människor som är föremål för den rättspsykiatriska vården. De patienter som deltagit i avhandlingens två delstudier kommer omväxlande att benämnas såsom deltagare eller patienter, för att åstadkomma en varierande och läsvänlig text.

    [2] I denna avhandling anser begreppet personal den personalkategori som arbetar med omvårdnad inom en rättspsykiatrisk kontext. Det är främst sjuksköterskor och skötare som inkluderas i begreppet omvårdnadspersonal inom den vetenskapliga litteraturen. Sjuksköterskor benämns såsom registered mental health nurses eller registered nurses. Övrig omvårdnadspersonal tituleras till exempel som nurse manager eller assistant nurses. När begreppen personal, vårdare eller omvårdnadspersonal används i denna avhandling avses då den personalkategori som arbetar närmast patienten med omvårdnad och ingen åtskillnad kommer att göras mellan de båda yrkesgrupperna. Personalen som deltar i delstudie IV har omväxlande kallats för personal eller vårdare.

    Aim: To explore if patients admitted to forensic psychiatric care decreased their assessed risk for violence over time, to identify patients who decreased their assessed risk for violence exceptionally well (30% or more) on the Clinical (C) and Risk management (R) scales in the (HCR-20), and to compare them in terms of demographic data.

    Methods: The HCR-20 risk assessment instrument was used to assess the risk for violence in 267 patients admitted to a Swedish forensic psychiatric clinic between 1997 and 2010. Their assessments at admission were compared with a second, and most recent, risk assessment.

    Results: The risk for violence decreased over time. Demographic criteria had no impact on differences on decreased risk. Only two factors, namely gender and psychopathy showed a difference. Risk factors associated with stress and lack of personal support were the items that turned out to be the most difficult to reduce.

    Conclusion: The results show that risk prevention in forensic care does work and it is important to continue to work with risk management. The study highlights the importance of a careful analysis of the patient's risk for violence in order to work with the patient's specific risk factors to reduce the risk.

    Empirical studies have rarely investigated the association between improvements on dynamic risk and protective factors for violence during forensic psychiatric treatment and reduced recidivism after discharge. The present study aimed to evaluate the effects of treatment progress in risk and protective factors on violent recidivism. For a sample of 108 discharged forensic psychiatric patients pre- and posttreatment assessments of risk (HCR-20) and protective factors (SAPROF) were compared. Changes were related to violent recidivism at different follow-up times after discharge. Improvements on risk and protective factors during treatment showed good predictive validity for abstention from violence for short-(1 year) as well as long-term (11 years) follow-up. This study demonstrates the sensitivity of the HCR-20 and the SAPROF to change and shows improvements on dynamic risk and protective factors are associated with lower violent recidivism long after treatment.

    The current study presents an empirical evaluation of the performance of the third version of the HCR-20. This prospective research project was conducted using a sample of 106 offenders and psychiatric patients who were transitioning out of institutions and into the community. Results provided strong support for the bivariate associations between the ratings of the presence and relevance of risk factors, as well as summary risk ratings, and violence at 4 to 6 weeks and 6 to 8 months. Although relevance ratings did not add incrementally to presence ratings, summary risk ratings added incrementally to both. Findings were not moderated by either sub-sample or gender. Version 3 of the HCR-20 was highly correlated with Version 2. Associations with violence were comparable between instruments. Findings were supportive of these basic elements of Version 3. © 2014 International Association of Forensic Mental Health Services.

    Abstract [en]

    The present study evaluated the utility of the Chinese version of the Revised Controlling Behaviors Scale (C-CBS-R) as a measure of controlling behaviors in violent Chinese intimate relationships. Using a mixed-methods approach, in-depth, individual interviews were conducted with 200 Chinese women survivors to elicit qualitative data about their personal experiences of control in intimate relationships. The use of controlling behaviors was also assessed using the C-CBS-R. Interview accounts suggested that the experiences of 91 of the women were consistent with the description of coercive control according to Dutton and Goodman's conceptualization of coercion. Using the split-half validation procedure, a receiver operating characteristics (ROC) curve analysis was conducted with the first half of the sample. The area under the curve (AUC) for using the C-CBS-R to identify high control was .99, and the cutoff score of 1.145 maximized both sensitivity and specificity. Applying the cutoff score to the second half gave a sensitivity of 96% and a specificity of 95%. Overall, the C-CBS-R has demonstrated utility as a measure of controlling behaviors with a cutoff score for distinguishing high from low levels of control in violent Chinese intimate relationships.
