    Abstract [sv]

    Introduktion: Medling med anledning av brott har funnits i Sverige sedan 1980- talet då ungdomsbrottsligheten betraktades som ett ökande samhällsproblem. År 1997 fick Brottsförebyggande rådet (Brå) i uppdrag av regeringen att stötta och främja olika medlingsprojekt runt om i Sverige och idag är medling lagstadgat, bland annat ska alla kommuner i Sverige sörja för att kunna erbjuda medling till samtliga lagöverträdare under 21 år. Metod: Studien var av kvalitativ ansats och utfördes genom åtta semistrukturerade telefonintervjuer med medlingssamordnare i Sverige. Respondenterna representerade fyra olika kommunstorlekar och intervjumaterialet analyserades genom en deduktiv innehållsanalys. Syfte: Syftet med studien var att belysa förutsättningar och begränsningar med att bedriva medlingsverksamhet i svenska kommuner. Resultat: Den främsta förutsättningen för att kunna bedriva medlingsverksamhet var medlingssamordnares uppfattningar om medlingens positiva påverkan på brottsdrabbade och gärningspersoner. Bristfälligt ärendeflöde, avsaknaden av nationella riktlinjer om hur arbetet borde organiseras samt bristande samverkansrutiner med andra aktörer uppgavs som betydande begränsningar. Diskussion: Medlingsverksamheter i Sverige organiserades på olika sätt i olika kommuner vilket kunde bero på den ramlagstiftning som finns om medling. Det varierande ärendeflödet kunde ses som ett resultat av bristande kunskap om medlingens syfte och effekter. För att medlingsverksamheterna ska bedrivas rättssäkert föreslog författarna till den aktuella studien ett tydligt centralt ansvar för verksamheterna samt att riktlinjer och direktiv tydliggörs. Författarna ansåg också att en fungerande medlingsverksamhet i Sverige var viktig då tidigare forskning rapporterat att medling som intervention har minskat återfall och ökat välmående hos såväl gärningspersoner som brottsdrabbade.

    Abstract [en]

    Aim:

    The aim of the current study was to examine the association of personality,

    neighbourhood, and civic participation on the level of perceived social support if needed.

    Methods:

    The sample consists of a total of 35,797 men (16,035) and women (19,762) drawn

    from the Nord-Trøndelag Health Study 3 (HUNT3), aged 20-89, with a fully completed short

    version of the Eysenck Personality Questionnaire (EPQ) including a complete response to

    questions regarding perceived social support. A multinomial logistic regression model was

    used to investigate the association between the three-category outcomes (high, medium, and

    low) of perceived social support.

    Results: The Chi-square test detected a significant (p <

    0.001) association between personality, sense of community, civic participation, self-rated

    health, living arrangement, age groups, gender, and perceived social support except between

    perceived social support and loss of social network in which no significance was found. The

    crude and adjusted multinomial logistic regression models shows relation between medium

    and low scores on perceived social support, personality, and sources of social support.

    Interactions were observed between gender and self-rated health.

    Conclusions: There is an

    association between the level of perceived social support and personality, sense of

    community in the neighbourhood, and civic participation. Even if the interaction

    between men and self-reported health decreases the odds for low and medium social

    support, health professionals should be aware of men with poor health and their lack of

    social support.

    Abstract [en]

    Background: To explore if health related quality of life(HRQoL) increased after traditional yoga(TY), mindfulness based cognitive therapy(MBCT), or cognitive behavioral therapy(CBT), in patients on sick leave because of burnout. Methods: Randomized controlled trial, blinded, in ninety-four primary health care patients, block randomized to TY, MBCT or CBT (active control) between September 2007 and November 2009. Patients were living in the Stockholm metropolitan area, Sweden, were aged 18-65 years and were on 50%-100% sick leave. A group treatment for 20 weeks, three hours per week, with homework four hours per week. HRQoL was measured by the SWED-QUAL questionnaire, comprising 67 items grouped into 13 subscales, each with a separate index, and scores from 0 (worse) to 100 (best). SWED-QUAL covers aspects of physical and emotional well-being, cognitive function, sleep, general health and social and sexual functioning. Statistics: Wilcoxon's rank sum and Wilcoxon's sign rank tests, Bonett-Price for medians and confidence intervals, and Cohen's D. Results: Twenty-six patients in the TY (21 women), and 27 patients in both the MBCT (24 women) and in the CBT (25 women), were analyzed. Ten subscales in TY and seven subscales in MBCT and CBT showed improvements, p <0.05, in several of the main domains affected in burnout, e.g. emotional well-being, physical well-being, cognitive function and sleep. The median improvement ranged from 0 to 27 points in TY, from 4 to 25 points in CBT and from 0 to 25 points in MBCT. The effect size was mainly medium or large. Comparison of treatments showed no statistical differences, but better effect (small) of both TY and MBCT compared to CBT. When comparing the effect of TY and MBCT, both showed a better effect (small) in two subscales each. Conclusions: A 20 week group treatment with TY, CBT or MBCT had equal effects on HRQoL, and particularly on main domains affected in burnout. This indicates that TY, MBCT and CBT can be used as both treatment and prevention, to improve HRQoL in patients on sick leave because of burnout, reducing the risk of future morbidity.

    Abstract [en]

    Purpose

    The purpose of this paper is to explore the gaps between experienced working conditions (WCs) and the perceived importance of these conditions in relation to subjective health in Swedish public sector workplaces.

    Design/methodology/approach

    In total, 379 employees answered questions concerning WCs and health. Nine WC areas were created to measure the gap between the experienced WCs and the perceived importance of each condition. These WC areas were: physical work environment, social relationships, communication, leadership, job control, recognition, self-development, workplace culture and work/life satisfaction. Subjective health was measured using mental ill health, well-being and general health.

    Findings

    The results indicated relatively large gaps in all nine WC areas. Leadership, physical work environment and work/life satisfaction in particular seemed to be problematic areas with relatively large gaps, meaning that employees have negative experiences of these areas while perceiving these areas as very important. Additionally, all WC areas were significantly related to subjective health, especially regarding mental ill health and well-being; the larger the gaps, the worse the subjective health. The WC areas of work/life satisfaction, self-development, social relationships, communication and recognition had the highest relationships and model fits. This indicates that it is most problematic from an employee’s point of view if there are large gaps within these WC areas.

    Originality/value

    This study improves the understanding of workplace health by exploring the gap between experienced WCs and the perceived importance of these conditions.

    Osäkra anställningsformer som timanställningar har under de senaste åren ökat i Sverige. Personer med sådana anställningsformer befinner sig i en mer utsatt situation i jämförelse med tillsvidareanställda. Timanställda har ofta begränsat inflytande över sitt arbete och små utvecklingsmöjligheter. En anställningsform som timanställning har även stor inverkat på privatlivet, både vad gäller arbetstider och varaktighet.

    Syfte: Syftet var att beskriva vilka faktorer som påverkar arbetstillfredsställelsen och den psykosociala arbetsmiljön hos ett urval timanställda på ett företag i Mellansverige.

    Metod: Genom ett induktivt angreppssätt, en traditionell forskningsprocess och en kvalitativ datainsamlingsmetod har sex stycken semistrukturerade intervjuer genomförts.

    Resultat: De kategorier som påverkade Arbetstillfredsställelse var: Förmåner ger en positiv upplevelse av arbetsplatsen, Lön ger uppskattning, Variation i arbetet och Trivsel på arbetsplatsen.

    De kategorier som påverkade den Psykosocial arbetsmiljö var Arbetet påverkar individen, Varierande arbetsdagar, Negativ inverkan på hälsan, Höga förväntningar men dålig feedback och Sociala relationer.

    Abstract [sv]

    Subjektiv livskvalitet är ett svårt begrepp att definiera och att mäta. Den har försökt illustreras som ett gap, mellan förväntningar hos individen och individens nuvarande erfarenhet. Eftersom den grundar sig på individuella upplevelser, kan den inte bedömas av någon annan än individen själv. Syftet med denna studie var att få kunskap om förväntningar och reella erfarenheter bland deltagare i den arbetslivsinriktade rehabilitering på Arbetsförmedlingen. Ansatsen var kvalitativ och insamlingen av data skedde via semistrukturerade intervjuer. På grund av endast tre deltagare, gjordes innehållsanalysen detaljerad och ledde till tretton kategorier med dubbelt så många subkategorier. För att göra resultatet överskådligt delades dessa in i fyra huvudkategorier: Individuella faktorer, Offentlig verksamhet, Regelverk och Arbetsmarknad. Resultatet visade att erfarenheter i rehabiliteringsprocessen påverkade deltagarnas upplevda livskvalitet. Då förväntningar gick att identifiera visade det sig att även de hade en påverkan på livskvaliteten.

    Abstract [en]

    OBJECTIVES: Premenstrual syndrome and premenstrual dysphoric disorder (PMS/PMDD) bother a substantial number of women. Homeopathy seems a promising treatment, but it needs investigation using reliable study designs. The feasibility of organizing an international randomized pragmatic trial on a homeopathic add-on treatment (usual care [UC] + HT) compared with UC alone was evaluated.

    DESIGN: A multicenter, randomized, controlled pragmatic trial with parallel groups.

    SETTINGS/LOCATION: The study was organized in general and private homeopathic practices in the Netherlands and Sweden and in an outpatient university clinic in Germany.

    SUBJECTS: Women diagnosed as having PMS/PMDD, based on prospective daily rating by the daily record of severity of problems (DRSP) during a period of 2 months, were included and randomized.

    INTERVENTIONS: Women were to receive UC + HT or UC for 4 months. Homeopathic medicine selection was according to a previously tested prognostic questionnaire and electronic algorithm. Usual care was as provided by the women's general practitioner according to their preferences.

    OUTCOME MEASURES: Before and after treatment, the women completed diaries (DRSP), the measure yourself concerns and well-being, and other questionnaires. Intention-to-treat (ITT) and per protocol (PP) analyses were performed.

    RESULTS: In Germany, the study could not proceed because of legal limitations. In Sweden, recruitment proved extremely difficult. In the Netherlands and Sweden, 60 women were randomized (UC + HT: 28; UC: 32), data of 47/46 women were analyzed (ITT/PP). After 4 months, relative mean change of DRSP scores in the UC + HT group was significantly better than in the UC group (p = 0.03).

    CONCLUSIONS: With respect to recruitment and different legal status, it does not seem feasible to perform a larger, international, pragmatic randomized trial on (semi-)individualized homeopathy for PMS/PMDD. Since the added value of HT compared with UC was demonstrated by significant differences in symptom score changes, further studies are warranted.

    Abstract [en]

    Purpose – Small-scale enterprises are important for sustainable development in Europe and account for a significant proportion of private enterprises and their large contribution to employment. The overall research aim of the study is to explore workplace health management from the perspective of managers in small-scale enterprises (SSEs) in Norway and Sweden. Methodology – In-depth interviews with 18 managers in SSEs were conducted and a stepwise qualitative analysis was used. Findings – The findings are presented as two main patterns 1) Interorganisational dynamics and 2) Participative leadership. Managers discussed opportunities for workplace health management to foster solidarity and flexibility in the workplace, the potential of employees for self-governance, and a cultural environment at the workplace characterized by safety,trust, care, loyalty and humour. The managers employed a process-oriented communicato style, were all-rounders, and demonstrated dedicated and distinct management. Managers in small-scale enterprises were lonely problem-solvers and experienced high and conflicting work demands and work-family conflicts. Research limitations – The findings should be interpreted with caution concerning representation of small-scale enterprises generally. The enterprises were recruited from an intervention project focusing on workplace health management and might therefore have a positive attitude. Originality – This study adds important knowledge regarding the preconditions for creating health-promoting workplaces in SSEs, an area for which limited research exists. The findings provide insights and knowledge about managers’ possibilities and obstacles in workplace health management. The findings could be transferrable to management in similar contexts if managers develop more awareness and knowledge. Practical implications – The managers obtain recommended information about what to doand how to address workplace health management in SSEs.

    Samhället eftersträvar att förhindra våld. Tre ansatser har etablerats för att avgöra farlighet och för att bedöma risk för våld; den ostrukturerade professionella bedömningsansatsen, den aktuariska bedömningsansatsen, samt den strukturerade professionella bedömningsansatsen. HCR-20 är ett strukturerat professionellt bedömningsinstrument med hög validitet och interbedömarreliabilitet i bedömning av risk för våld. HCR-20 genomgår en evidensbaserad uppdatering och den reviderade versionen skall utvärderas - syftet med studien var därför att undersöka interbedömarreliabilitet för den tredje versionen av HCR-20 [HCR-V3]. Vid Rättspsykiatriska regionkliniken i Sundsvall riskbedömdes 35 patienter som inskrivits för tvångsvård under februari 2010 till oktober 2011. Interbedömarreliabilitet undersöktes för grad av överensstämmelse mellan bedömare samt också gentemot Golden Standard, bedömarnas gemensamma bedömning av patienten. Överensstämmelsen i HCR-V3bedömningarna beräknades på huvudfaktorer och på relevansfaktorer med Kendall’s τb, Cohen’s Κ, och ICC(1,2). HCR-V3 påvisade hög interbedömarreliabilitet med hög grad av överensstämmelse mellan användarnas bedömning av patientens risk för våld. Det var högre interbedömarreliabilitet för H-delen och C-delen än för R-delen. Överensstämmelsen mellan enskilda bedömare och Golden Standard var högre än överensstämmelsen mellan två bedömare, och huvudfaktorerna indikerade på högre interbedömarreliabilitet än relevansfaktorerna. Utifrån resultatet drogs slutsatsen att riskbedömningsinstrumentet HCR-V3 förmodligen inte skulle bli mer användarvänligt med relevansfaktorer, men att huvudfaktorerna ger hög interbedömarreliabilitet och att HCR-V3 därmed kan vara lämplig i klinisk verksamhet för att förhindra våld.

    Work For You är privat fristående aktör som Arbetsförmedlingen upphandlat rehabiliteringstjänster av. Syftet med denna studie var att studera och analysera hur deltagarna på Work For You upplevt rehabiliteringstjänsten de ingått i. Ett annat syfte var att även ta reda på om rehabiliteringstjänsten haft någon inverkan på deltagarnas möjlighet att återgå till arbetsmarknaden. Studien utgick ifrån fyra deltagares upplevelser av insatsen.

     

    Studien har en kvalitativ induktiv ansats. Resultatet visar att samtliga deltagare upplevt att insatsen haft en positiv inverkan på dem och de anser att de fått den hjälp de ansett sig vara i behov av. Deltagarna upplevde att de fått hjälp i ett helhetsperspektiv, kommit ut i en anpassad arbetsträning och återfått en tro på sin egen förmåga. Man har även parallellt med de arbetslivsinriktade insatserna haft möjlighet att träna på det sociala samspelet, få kunskap om anpassningsmöjligheter och fått ett ökat självförtroende. Några av de som deltagit i intervjun har nämnt att insatsen för dem varit en lösning eller lett till ett bättre mående, där de fått ett stärkt självförtroende och känner hopp. Samtliga har även varit ute på arbetsträning.

    Current research indicates that stress problems primarily could be conceptualized as deficienciesin recovery and recuperation between stress periods. Accordingly, interventions should put moreemphasis on this aspect. A group based intervention program focusing exclusively on recoverybehavior in everyday life was evaluated in this quasi-experimental, waiting-list control groupstudy, where the control group was also treated in a second phase. Thirty-two self-referred femalesubjects, considering themselves in need of treatment for stress related health problems, wereavailable for analyzes. Fifteen of these constituted the first phase treatment group (INT), while theremaining 17 subjects were placed on waiting list (WLC). Adding a few late applicants leaved 20subjects later treated in the second intervention phase. Significant and clinically meaningful positiveeffects emerged in the INT—compared to the WLC-group on recovery behaviors, stress—andrecovery experiences, as well as on burn-out symptoms, worry, anxiety and depression. Secondaryanalyzes of all treated subjects indicated that the positive change the primary clinical endpointwas predicted by the increase in frequency of recovery behaviors and by the decrease in the worrylevel. Thus, the present intervention model merits further research with more rigorous experimentaldesign as well as with follow-up assessments.
