Abstract [sv]

Bakgrund: Palliativ vård kräver stora omvårdnadsinsatser såväl som medicinska insatser, därigenom utgör distriktssköterskan och läkaren centrala delar av det palliativa teamet. Gles landsbygd präglas av stora geografiska avstånd och kan innebära att de olika professionerna befinner sig långt ifrån varandra, vilket kan medföra en utmaning för samarbetet. Syftet med denna studie var att beskriva distriktssköterskors upplevelser av samarbetet med ansvarig läkare vid palliativ hemsjukvård i gles landsbygd. Metod: Semistrukturerade intervjuer utfördes med tio distriktssköterskor. Materialet bearbetades genom induktiv innehållsanalys. Resultat: Distriktssköterskornas upplevelser beskrivs i det övergripande temat Erfarenheter av att agera som vårdtagarens företrädare, som mynnar i två kategorier. Kategorin Positiva upplevelser av samarbete genererade underkategorierna Stolthet, Trygghet, Stöttning och Ömsesidighet. Kategorin Negativa upplevelser av samarbete frambringade underkategorierna Otillräcklighet, Frustration och Tidsnöd. Diskussion: Distriktssköterskor och läkares samsyn och samstämmighet, eller brist på detsamma, har betydelse för hur samarbetet upplevs. När läkaren delar distriktssköterskans holistiska förhållningssätt genererar det positiva upplevelser, medan samarbetet upplevs negativt när läkaren inte tar beslut i enlighet med vad distriktssköterskan anser gagnar vårdtagaren. Slutsats: Upplevelserna är sannolikt inte unika för denna kontext utan kan möjligen förklara det interprofessionella samarbetet även i fler sammanhang där omvårdnadsprofessionen och den medicinska professionen möts. För optimering av samarbetet krävs förståelse för hur det upplevs, varvid motsvarande kartläggning av läkares upplevelser föreslås för framtida studier.

Abstract [en]

Background: Palliative care largely requires nursing as well as medical care, thus the district nurse and the physician are central parts of the palliative team. Sparsely populated rural areas are characterized by great geographical distances and can embed that the various professions are far from each other, which can pose challenges for cooperation. The aim of this study was to describe district nurses’ experiences of the collaboration with responsible physicians at palliative home care in rural areas. Method: Semi-structured interviews were conducted with ten district nurses. Interviews were processed by inductive content analysis. Findings: District nurses’ experiences is described in the overall theme Experiences of acting as the patients’ advocate, ending in two categories. The category Positive experiences of collaboration generated the subcategories Pride, Security, Support and Reciprocity. The category Negative experiences of collaboration produced the subcategories Insufficiency, Frustration and Time constraints. Discussion: District nurses and physician’s consensus and coherence, or lack thereof, have a bearing on how the collaboration is perceived. When physicians shares the district nurse’s holistic approach, it generates positive experiences, while the collaboration is experienced negatively when physician’s decisions don’t comply with what the district nurse considers to be beneficial to the patient. Conclusion: The experiences are probably not unique for this context but can make it possible to explain the interprofessional collaboration in several contexts where the nursing profession and the medical profession meet. Optimizing the collaboration requires an understanding of how it is experienced, whereby a corresponding survey of physician’s experiences is proposed for future studies.
